HAR §17-896-17 — Staff-child ratio
Please submit an update DHS 954 Child Care Center Staff and Volunteer List and a DHS 957 Staff Sequence Form to show that each group has proper staff-child ratio's and sequence. In the 4/5/6 group, aides cannot constitute for more than one-third of the required staff child ratio. For 70 children, 4 staff are required. One-third of this is one. Therefore, there shall be at minimum, three (3) program leaders and one (1) aide for this group. Currently, there is one (1) qualified program leader and four (4) aides. Both forms are enclosed. As a reminder, staff child ratio is one 1 staff member for every 20 children and aides cannot constitute for more than one-thrid of the required staff child ratio. Correction: Worker received updated DHS 954 Child Care Center Staff and Volunteer List and a DHS 957 Staff Sequence Form that shows each group has proper staff-child ratio and sequence.

HAR §17-896-3 — Employment history check
Please submit employment history checks for the following employees: XXXXX As a reminder each employee must have a DHS 959 and 959A completed for the past 3 years of employment. Employment History Instructions: Individuals shall complete the form DHS 959A "Self Certification of Employment" and list the places of employment within the past three (3) years including periods of unemployment, self-employment, and periods of school attendance. For every place of employment listed, complete and sign Part I of the DHS 959 "Employment History Clearance Form" (EHC) and have the former and/or current employer complete and sign part II. Please complete the following information on the form DHS 959A: Date EHC mailed and Date EHC returned. Keep copies of all forms mailed out to former employers. If EHC forms are not returned after the first try, mail out the copy again and log the second date it was mailed. HAR §17-896-16 — Staff training, experience, and personal qualifications
17-896.1-35 (b) Each caregiver, substitute, and volunteer within forty-five days of hire, shall complete initial training that is approved by the department in health and safety standards. Please have the following staff submit their initial health and safety assessments to PATCH immediately: XXXXX Failure to do so will result in the individuals being excluded from providing child care until the training requirement is met.
