HAR §17-892.2-35 — Caregivers shall complete at least 16 hours of ongoing training in at least 2 of the department approved topic areas

All staff shall have 16 hours of annual health and safety trainings; substitutes shall have 10 hours of training. Staff who do not meet this requirement may be excluded in the staff-child ratio, which may affect your license to operate the group child care program. Note: ALL staff is required to meet this requirement annually . The department advises each provider to plan accordingly for timely staff development trainings as well as time that these trainings can be recorded timely. We are reminding you that all caregiving staff need at least sixteen (16) hours of department approved training on an ANNUAL basis in at least two topic areas and substitutes need at least 10 hours of training. If they do not complete this requirement we may have to exclude these staff members and they will not be able to count towards the staff-child ratio until they do, which may affect your license to operate a Group Child Care Center (GCC) . If training has been completed, please immediately provide the certificates to PATCH. If the staff has more training to do, please communicate a plan on how they shall meet the training requirement timely.

HAR §17-892.2-67 — Food service establishment permit expires: <date>

Provide the current food establishment permit which allows you to serve snacks. Note: since permits are issued annually by the Department of Health, please provide this current permit to show that the program can serve foods.

HAR §17-892.2-62 — Written procedures for infection control to prevent transmission of blood-borne pathogens

HAR §17-892.2-61 — Written evidence of fire warning device or system has been inspected initially and annually and approved by a by fire inspector. Report dated: <date>

Provide the current fire inspection report for the Junior Kindergarten classroom. To meet this licensing requirement, any and all findings by the fire department shall be addressed. If there are findings, a new report shall be provided to show the findings have been addressed.

HAR §17-892.2-61 — Plan relating to fire emergencies hall be approved by fire inspector, and the facilities health consultant

Provide your current emergency preparedness plan. Once received, Worker will review the plan to ensure that it meets current requirements.

HAR §17-892.2-56 — Written health policies for staff for personal health care and control for the spread of communicable diseases

Provide the staff procedures handbook to show that you have current policies for personal health care and control for the spread of communicable diseases. If you need assistance, please contact worker, Seng Lee-Ray.

HAR §17-892.2-48 — Annual nutrition review dated: <date>

Provide a menu review of all meals served to the children in the Junior Kindergarten classroom to show that it meets the nutritional requirements of children in care. Note: menu reviews of all foods served by Hanahauoli School has to be conducted annually by a registered license nutritionist. Please make sure to show the department that this is done annually going forward. The Department of Health provides a free menu review for licensed child care providers. HAR §17-892.2-43 — One caregiver with current pediatric first aid certificate and child CPR during operating hours

Provide the current first aid and child cardiopulmonary resuscitation (CPR) certificates for the Junior Kindergarten staff.

HAR §17-892.2-40 — Written evidence of agreement or contractual agreement for health consultation (private physician, non-profit health organization, community health consultation service, or health advisory board)

Provide a letter of agreement to show that you have a health consultant for the Junior Kindergarten program. The health consultant is also required to review your health policies (for the Junior Kindergarten program and it's staff) periodically. In the letter, please have the health consultant state that they have reviewed the licensed program's (Junior Kindergarten) health policies and that they are satisfactory.

HAR §17-892.2-17 — Log of emergency preparedness drill practices

Provide a log of the program's emergency evacuation drills to show that emergency evacuation drills are conducted at regular intervals for the Junior Kindergarten program.

HAR §17-892.2-12 — Evidence of liability coverage

Provide the current certificate of insurance to show the program has liability coverage. Note: please provide this certificate at the start of each coverage period.

HAR §17-892.2-3 — Background checks according with chapter 17-801 (criminal history, sex offender registry, child abuse/neglect, adult abuse perpetrator, employment history check, and other information)

Annual background checks have expired or shall expire soon for all staff. Please provide the completed DHS 948 Authorization for Background Check and to Release Information form, so the department can conduct annual background checks. This requirement shall be met when all checks are completed, and there are no findings. HAR §17-892.2-17 — Written permission from parents to allow child to have contact with pets or other animals

Provide the written permission form that parents sign to allow children who are in the Junior Kindergarten program to have contact with pets or other animals.

HAR §17-892.2-11 — Ages of children accepted

The current parent handbook has not been provided to us. Please submit your current preschool handbook for review. Once received, worker will determine if the statement of operation policies are covered and if they meet the licensing requirements.

HAR §17-892.1-36 — Sanitation inspection
