HAR §17-891.2-90 — Written practices and policies reviewed with staff, volunteers, and/or caregivers upon hire, start of care, or start of residence and on an annual basis

Safe sleep training has expired for XXXXX. Please do this training immediately and provide the certificate to PATCH for recording. Failure to do so, will result in not being able to care for children under 12 months of age. Reminder : Training for XXXXX expires in July 2026. Please make sure that XXXXX does this training timely.

HAR §17-891.2-3 — Background checks according with chapter 17-801 (criminal history, sex offender registry, child abuse/neglect, adult abuse perpetrator, employment history check, and other information)

Background checks expired for XXXXX and will be expiring for XXXXX. Please submit completed DHS 948 Authorization for Background Check and to Release Information forms immediately. HAR §17-891.2-15 — Admission policies that require sufficient information and instruction to be received from parents/guardians that enable caregiver to care for the child

During the visit, Worker reviewed 2 child records that were incomplete. Provider reported XXXXX has 1 full time and 3 part time children in care. Please inform Worker XXXXX when you have collected the child and family information and health information for all enrolled children.

HAR §17-891.2-11 — Admission of sick children or children with special needs

Please amend the statement regarding the non-discrimination and compliance with American with Disability Act (ADA) in your parent handbook. You can use this statement: It is this home's policy to not discriminate against persons with disabilities on the basis of disability, and to provide children and parents with disabilities an equal opportunity to participate in the home's programs and services, in compliance with state law and the federal Americans with Disabilities Act. Correction: The policy has been updated.

HAR §17-891.2-66 — Written disaster plan to cover fire, flood, or natural disaster or other emergencies posted

Please update your emergency disaster plan to include (communication with parents on) reunification and continuity of operations, how you will get non-mobile infants/toddlers out safely, and how you will accommodate children with disability, chronic medical needs, and special needs. Worker has emailed an emergency response sample plan to the provider. Your emergency disaster plan was required to be updated by September 30, 2024 to be in compliance with the new rules. If you do not correct this deficiency by 12/31/25, your license to operate an FCC may be affected as not having a current emergency disaster plan affects the safety of your enrolled children. Correction: Provider updated the emergency preparedness plan to include the above.

The policy handbook shall be revised to meet the new requirements of Hawaii Administrative Rules 17-891.2. Please provide a copy of the amended policy handbook. The Department enclosed the attachments that were originally mailed to you on May 31, 2024 for your information regarding the new requirements. Correction: Policy handbook amended according to HAR 17-891.2.
