Annual Background Check Clearances for one (1) household member XXXXX is expired and one (1) staff member's XXXXX is soon to be expired. CCL Worker received DHS 948 Authorization for Background Check and to Release Findings for XXXXX and are awaiting the results of the clearances. Once we receive satisfactory results of the clearances, this deficiency will be corrected. Please submit the annual on-going Health & Safety (H & S) trainings to PATCH immediately for XXXXX, as the training period is expired. We are reminding you that XXXXX are required to complete 16 hours of H & S training on an ANNUAL basis. To date, our records indicate that XXXXX have only completed one (1) hour of Safe Sleep training, which also qualifies towards one (1) hour of H & S training. Fifteen (15) additional hours of other training are still required to complete the initial training period from August 30, 2024 through August 29, 2025. Please note that subsequent training periods require sixteen (16) hours of H & S training on an ANNUAL basis. The current training period for August 30, 2025 through August 29, 2026 requires sixteen (16) hours of H & S training. Documentation verifying completion of the required H & S training must be submitted to PATCH no later than June 30, 2026. If no substantial progress has not been made by that date, we may proceed with suspension of your Certificate of Registration, which will affect your authority to operate as a registered Family Child Care (FCC) provider.
