HAR §17-892.2-35 — Caregivers shall complete at least 16 hours of ongoing training in at least 2 of the department approved topic areas

Correction: The Worker confirmed that People Attentive to Children (PATCH) has received and verified the on-going Health & Safety trainings for XXXXX.

HAR §17-892.2-3 — Inspection of facility, activities, records, staff

The Worker conducted a visit on September 9, and October 20, 2025 to The Early Education Center to inspect the program's new facility located at the Mission Memorial Building located at 550 South King Street. We will need to return to conduct an observation of the children in care in addition to verifying that the staff-child ratio and staff employment sequence are being met, as well as observing activities and verifying records. Correction: Child Care Licensing (CCL) Worker conducted an unannounced drop-in visit to The Early Education Center on December 15, 2025 to observe the children in care, and verify that the staff-child ratio is being met. During the visit, the children were observed during indoor and outdoor playtime. The Worker also verified that the roster of enrolled children, attendance records, and children's health records were accounted for. At the time of the visit, there were 29 children in attendance.

HAR §17-892.1-3 — Inspection of facility, activities, records, staff

All providers shall comply with the new Group Child Care Centers Hawaii Administrative Rules (HAR) effective December 31, 2024. Please note the following: HAR §17-892.2-35 (e) Staff training: Each caregiver shall complete on-going (Health & Safety) training within twelve months from the date the initial training was received and recorded by the department, and annually thereafter. Please submit the on-going Health & Safety trainings to PATCH for XXXXX. 16 hours of training is required. Note: Failure to complete the required training will result in the employees being excluded from providing child care until they complete the required training. Correction: The Worker verified that staff member XXXXX has completed her on-going health and safety trainings, and submitted them to PATCH.

HAR §17-892.1-3 — Employment history check

Please submit the employment history check clearances (DHS 959 Employment History Form and DHS 959A Self-Certification of Employment form) for XXXXX. Employment History Instructions: Required individuals shall complete the form DHS 959A "Self Certification of Employment" and list the places of employment within the past three (3) years including periods of unemployment, self-employment, and periods of school attendance. For every place of employment listed, complete and sign Part I of the DHS 959 "Employment History Clearance Form" (EHC) and have the former and/or current employer complete and sign part II. Please complete the following information on the form DHS 959A: Date EHC mailed and Date EHC returned. Keep copies of all forms mailed out to former employers. If EHC forms are not returned after the first try, mail out the copy again and log the 2nd date mailed. If forms are not returned after the second try, we will accept the self-certification. HAR §17-892.1-17 — All caregivers meet qualifications

Please contact PATCH to register XXXXX with the Hawaii DHS Early Childhood Registry. Also, please submit the current Child Development Associate Credential certificates to PATCH for XXXXX. Correction: The Worker verified with PATCH that XXXXX are now registered with the Hawaii DHS Early Childhood Registry. Also, the Worker as well verified with PATCH that XXXXX have their current CDA certificates.

HAR §17-892.1-18 — Staff-child ratio

During the visit, three teachers were being used in the staff-child ratio, but currently have an expired Child Development Associate Credential (CDA): XXXXX. Please submit their current CDA certificates to PATCH. Correction: The Worker verified with PATCH that XXXXX have their current CDA certificates.

HAR §17-892.1-29 — Physical exam clearance

Please submit evidence of a satisfactory physical examination (DHS 984), taken within one (1) year of their date of hire, for XXXXX.
