During the visit, the Worker observed in the outside play area a small picnic table that contained a rusted side piece. Area manager was notified of the hazard. Please submit a photo once the rusted side piece on the picnic table has been replaced. Correction: Worked received email from Area Manager containing a photo of the picnic table without the rusted piece attached.

We have received the DHS 948 Authorization for Background Check and to Release Information forms for XXXXX and are awaiting the results of the clearances. Once we receive satisfactory results of the clearances, this deficiency will be corrected. Correction: Currently, background clearance checks have been completed for all staff members: XXXXX.
