At the time of the visit, the facility does not have a current menu review on file with DHS. Correction: On February 4, 2026, Samuel Foulchag of the HCCNP emailed the worker a copy of the facility's updated Menu Review.

HAR §17-892.2-61 — Written evidence of fire warning device or system has been inspected initially and annually and approved by a by fire inspector. Report dated: <date>

At the time of the visit, the facility did not have a current satisfactory fire inspection report on file with DHS. Correction: Kenneth Bogowitz of the C&C Fire Prevention Bureau emailed the worker a copy of the Satisfactory Fire Inspection report, dated March 12, 2026.

At the time of the visit, there was no medical history on file for 3 of the 5 staff members. Correction: The director faxed all missing medical forms to the worker.

At the time of the visit, there was no medical history on file for 3 of the 5 staff members. Correction: The director faxed all missing medical forms to the worker.

HAR §17-892.1-6 — Insurance coverage policy

At the time of the visit, there was no current liability insurance on file. The director stated that she will work on obtaining liability insurance as soon as possible. Correction: The director emailed the worker a copy of the liability insurance effective February 27, 2024 to February 27, 2025.

HAR §17-892.1-20 — Health record on each child

The worker reviewed the children's records and found that one child was missing their medical records. Correction: The worker received an email from the director with the child's missing medical records, which were up-to-date.

HAR §17-892.1-26 — Annual nutrition review

The director informed the worker that they have been in contact with Sam Foulchag, of the Hawaii Child Care Nutrition Program. However, no visit for the menu review has been scheduled at the time of the visit. Correction: Samuel Foulchag sent the worker an updated menu review.
