HAR §17-892.2-63 — Furniture, equipment and toys shall be sturdily constructed with minimal hazards

Director to repair rusting on sections of the climber and tent post in the outdoor space. Correction: Director confirmed that they will not use the dome structure until the rusted spots are repainted; hoping to complete painting on March 31, 2026. Barriers placed in front of the tent posts until they can treat and paint all posts.

HAR §17-892.2-3 — Background checks according with chapter 17-801 (criminal history, sex offender registry, child abuse/neglect, adult abuse perpetrator, employment history check, and other information)

Director to submit DHS 948 consent form (annual background check) for one worker as discussed. Correction: Worker received completed DHS 948 consent form.

HAR §17-892.2-61 — Written evidence of fire warning device or system has been inspected initially and annually and approved by a by fire inspector. Report dated: <date>

Director to contact MFD for an annual fire inspection and submit a copy of a completed report. Fire extinguishers last serviced in March 2025. Correction: Per director, fire inspection was completed on February 24, 2026; however, MFD has not issued an inspection report. Director will forward the completed report when received.

HAR §17-892.1-29 — Physical exam clearance

Director to submit completed medical report form for new staff (physical exam). Correction: Worker received completed medical form for new hire; on file.

HAR §17-892.1-3 — Employment history check

Director to submit completed employment history check and self certification of employment to the department for new hire. Correction: Worker received completed employment history clearance checks for new hire; on file.

HAR §17-892.1-29 — TB clearance

Director to submit verification of TB clearance/assessment for new staff. Correction: Worker received verification of TB assessment for new staff; on file.
