    HAR §17-892.2-35 — Caregivers shall complete at least 16 hours of ongoing training in at least 2 of the department approved topic areas

    XXXXX's annual health and safety training period expired on 01/02/2026. Currently, she has 10.25 out of 16 hours credited. The Worker received an email from XXXXX on 01/02/26 confirming that XXXXX training certificates were submitted to PATCH on 11/24/25. As such, once the hours have been updated in the system and no other staff have expired health and safety training periods, this deficiency will be satisfied. Additionally, XXXXX health and safety training period will be expiring soon. Currently, he has 8.75 out of 16 hours credited and his training period will expire on 02/08/26. In the same email from 01/02/26, XXXXX confirmed that XXXXX certificates were submitted to PATCH on 11/18/25. As a courtesy reminder, please ensure that all 16 hours of health and safety training certificates are submitted to PATCH around 2 months prior to each employee's training expiration date to avoid additional deficiencies. PATCH is currently estimating 6 weeks to process on-going training certificates. Correction: The Worker received an email on 1/6/26 from XXXXX at PATCH confirmation that these staff's training certificates have been submitted, verified, and scheduled for input. As such, the Worker accepted the email as confirmation of completion of training and has verified that all staff are currently in good-standing with health and safety training.
