Documentation must include the applicants job title and description of his/her regular duties, confirmation of employment dates, and level of job performance. The employer/owner/operator must make at least three attempts to obtain employment history information. Failed attempts to obtain employment history must be documented in the personnel file and include date, time, and the reason the information was not obtained. An employment history verification was not documented or incomplete. Sec. 7-32, Ordinance: Before the first day of employment at a child care facility, all child care personnel, excluding temporary substitutes, must have on file at the child care facility a signed statement from a licensed physician or authorized agent of the Florida Department of Health attesting that the person is in good health in order to care for children in a child care facility setting. The health assessment statement shall be documented on a form prescribed by the local licensing agency and updated at least every two years. Child Care personnel did not have a current health statement on file. Comments: At the time of inspection, [1] personnel file did not contain a No.8 form. Personnel resolved by providing No.8 form Compliance Comments Technical assistance: CCF Handbook, Section 7.4 AND (F), "Records must be maintained and kept current on all child care personnel, as defined by Section 402.302(3), F.S. These records shall be on-site, available for review by the licensing authority." --and-- "Prior to beginning volunteering in a child care facility, a CF- FSP 5217, Volunteer Acknowledgment which is incorporated by reference in 65C-22.001(7)(e), F.A.C., and may be obtained from the Departments website www.myflfamilies.com/childcare, must be completed and on file at the child care facility for the volunteer. Written documentation of volunteer hours must be maintained at the facility for a minimum of 12 months and available for review by the licensing authority." 3.12(C), Handbook: All equipment, fences, and objects on the facilitys premises shall be free from sharp, broken and jagged edges, and properly placed to prevent overcrowding or safety hazards in any one area. Equipment in the outdoor space needs repair. (Section 2.1(A) Health and Safety, Page 3) Indoor/Outdoor Space [SR] Comments: At the time of inspection, the licensing monitor observed the outdoor play space in need of the following: -repair rusting found throughout playground apparatus. -replenish mulch throughout the playground -the grass is overgrown and needs to be cut -the fence has a horizontal gap measuring 6 inches. Horizontal gaps may not be more than 3 inches. Following progressive enforcement procedures, a Notice of Violation is being issued as this standard was previously cited out of compliance on 3/10/25. 4.1, Handbook: Child care personnel including volunteers who work 10 hours or more per month must successfully complete the departments training within 12 months from the date training begins. Training completion may not exceed 15 months from the date of employment in the child care industry in any licensed Florida child care facility. The facility did not have documentation of child care personnel's completion of required training on the training transcript in that [Part 1 incomplete]. 3.12(C), Handbook: All equipment, fences, and objects on the facilitys premises shall be free from sharp, broken and jagged edges, and properly placed to prevent overcrowding or safety hazards in any one area. Equipment in the outdoor space needs repair. Indoor/Outdoor Space [SR] Comments: At the time of inspection, licensing monitor observed a platform on the playground structure to be cracked and rusted. Slide attached to the platform is no longer secure and platform is observed to bow under body weight. Rust is noticeable in other platforms and appear to need tightening. A small climbing wall attached to the structure is missing one of the footholds. Licensing monitor has recommended that structure be inaccessible until repairs are made, or parts are replaced. Provider has requested an extension of the due date and will utilize secondary playground area. Technical assistance is provided regarding paint and overall appearance of structures. 7.1, Handbook: The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form Part A-1, B, or C, DH 680, which is incorporated by reference in 65C-22.001(7)(o), F.A.C., or the Religious Exemption from Immunization form, DH 681, which is incorporated by reference in 65C-22.001(7)(p), F.A.C., from the custodial parent or legal guardian. The Florida Certificate of immunization was expired for children in care.
