07. Background Screening Requirements FDCH/LFCCH Handbook, Section 4 (07-09)
The operator, employee, substitute, and/or volunteer did not have a CF-FSP 5337 Child Abuse and Neglect Reporting Requirements form signed annually. FDCH/LFCCH Handbook, Section 4.3, H Comments: During the time of inspection, the Licensing Specialist observed that the substitute did not have a current Child Abuse & Neglect form on file. CCF Handbook Section 4.3, H Background Screening Documents H. The operator, substitute, employee and/or volunteer must complete a CF-FSP 5337, Child Abuse and Neglect Reporting Requirements, incorporated by reference in 65C-22.001(7)(l), F.A.C., signed prior to initial licensure and annually thereafter. Compliance Comments During the time of inspection, the Owner advised the Licensing Specialist that there 3 employees. During the time of the inspection, the Licensing Specialist monitored 3 of the 3 employee records. REMINDER: 10 hours of annual in-service training must be completed by renewal, 10/10/2025.
