TA was provided you need to make at a minimum of 3 attempts on previous employers. Documentation must include the applicants job title and description of his/her regular duties, confirmation of employment dates, and level of job performance. Failed attempts to obtain employment history must be documented in the personnel file and include date, time, and the reason the information was not obtained. Daily attendance was not maintained to account for all children in care. FDCH/LFCCH Handbook, Section 8.4 A and B Comments: At the time of the inspection on 2/3/26, the provider that is listed in the supplemental stated they have 17 children enrolled. All childrens files were reviewed by the licensing specialist.On 2/6/26 another on site visit was conducted to check the files for the additional people that were added to the clearing house roster. The roster could not be pulled on the first visit. There were 11 children present and only 8 children signed in. The staff filled in the dates.
