There was evidence of build up of dirt, dust, or stains on fixtures, equipment/furnishings as evidenced by [diaper changing table]. Comments: At the time of the inspection, observed what appears to be a build of dust and greeting cards on the diaper changing table. The Operator removed the greeting cards, and cleaned and sanitized the changing table at the time of the inspection. T.A The diaper changing table must be cleaned and sanitized after each use.
