A health and safety deficiency having a moderate potential for harm to the children in care was observed. FDCH/LFCCH Handbook, Section 7.6, A Comments: During the time of inspection, Licensing Specialist observed a large plant located in the outdoor play area which had sharp, prickly thorns. Children could prick/harm their hands and fingers if touched.FDCH/LFCCH Handbook, section 7.6 Outdoor Time, Fencing and Play Area RequirementsA. Outdoor Play areas shall be clean, in good repair and free from litter, nails, glass, and other hazards. Compliance Comments During the time of inspection, the Licensing Specialist observed the swimming pool water to be clean/clear and appropriate barriers were in place to prevent the children from gaining access to the swimming pool. During the time of inspection, the Provider advised the Licensing Specialist that the swimming pool will not be utilized for the children that will be in care. The bedding available was not safe and poses a threat to the health, safety or well-being of a child in care. FDCH/LFCCH Handbook, Section 7.8, A, B and D Comments: During the time of inspection, Licensing Specialist observed a four-month-old infant sleeping in a playpen. Inside of the playpen with the sleeping infant were blankets, toys, diapers, wipes, and a string attached to a pacifier. Infant was also wearing a bib. Licensing Specialist observed Provider remove all of the items out of the playpen.This standard was brought back into compliance during the time of inspection.FDCH/LFCCH Handbook, section 7.8 Appropriate, Safe and Sanitary BeddingD. FDCH/LFCCH Handbook, Section 7.8, F Comments: During the time of inspection, Licensing Specialist observed several torn/ripped mats.FDCH/LFCCH Handbook, section 7.8 Appropriate, Safe and Sanitary BeddingA. A home must include a designated area where each child can sit quietly or lie down to rest or nap. Each child in care must be provided safe and sanitary bedding to be used when napping or sleeping. Linens must be washed at least once a week and more often if soiled or dirty. All bedding and linens must be thoroughly cleaned and sanitized at least once a week and before use by another child. A prescription or non-prescription medication, specifically [pain & fever medicine and hydrocortisone cream], was dispensed without written authorization from the custodial parent or legal guardian. FDCH/LFCCH Handbook, Section 7.24, A and L Comments: During the time of inspection, Licensing Specialist observed liquid acetaminophen and baby tiny cold tablets being stored in the refrigerator. Licensing Specialist did not observe written authorization to dispense medication. Provider advised Licensing Specialist that the medication has been in there for a long time and the child does not use it anymore. The Provider stated that she will return medication to parent at pick up. Licensing Specialist also observed prescription hydrocortisone cream stored in a childs bag in a bin. Provider advised Licensing Specialist that she did not know that parent had put the hydrocortisone cream into the childs bag. Licensing Specialist observed Provider discard hydrocortisone cream.This standard was brought back into compliance during the time of inspection.FDCH/LFCCH Handbook, section 7.24 MedicationThe family day care home or large family child care home is not required to give medication; however, if they choose to do so, the following shall apply: A. The operator must have written authorization from the custodial parent or legal guardian to dispense prescription and non-prescription medications. This authorization must be dated and signed by the custodial parent or legal guardian and must contain the childs name; the name of the medication to be dispensed; and the date, time and amount of dosage to be given. This record shall be initialed or signed by the child care personnel who gave the medication. Prescription and non- prescription medications that are used on an as needed basis require the parent/ legal guardian to provide additional documentation on the authorization form to describe symptoms that would require the medication to be given. The child care personnel must never administer a medication that is prescribed for one child to another child.I. Medication which has expired or is no longer being administered shall be returned to the custodial parent or legal guardian or discarded if the child is no longer enrolled in care at the home. The Florida Certificate of immunization was not acceptable in that: [ The form was not current (expired)].FDCH/LFCCH Handbook, Section 8.1 A and B Comments: During the time of inspection, Licensing Specialist observed 3 children with expired immunization records.FDCH/LFCCH Handbook, section 8.1 Health/Immunization Records A. The operator is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form, Parts A-1, B, and/or C, DH 680 (July 2010), or the Religious Exemption from Immunization form, DH 681 (July 2008), which are incorporated herein by reference in 65C-22.001(7)(o) and (p), F.A.C., from the custodial parent or legal guardian, within 30 days of enrollment. DH Form 680 and DH Form 681 may be obtained from the local health department. The parent/guardian of a child who has not received the age-appropriate immunizations prior to enrollment and who does not have documented medical or religious exemptions from routine childhood immunizations should provide documentation of a scheduled appointment or arrangement to receive immunizations. Toxic Substances and/or Hazardous materials including cleaning supplies, flammable products, and poisonous items were accessible to children in care. FDCH/LFCCH Handbook, Section 7.2 (Section 2.1 Health and Safety, Page 3) Comments: During the time of inspection, Licensing Specialist observed glass cleaner, air freshener, body sprays and lotions on a shelf located in the bathroom that the children use which was accessible to them. Provider removed the shelf along with the toxic substances out of bathroom and put it in an area that was no longer accessible to the children in care. This standard was brought back into compliance during the time of inspection. FDCH/LFCCH Handbook, section 7.2 Toxic Substances, Hazardous Materials and Poisonous Items A. All areas and surfaces accessible to children shall be free from toxic substances and hazardous materials/equipment/tools, including power tools, plastic bags, matches, candles, lighters, etc. These items, as well as knives, sharp tools, BB guns, pellet guns and other potentially dangerous hazards, shall either be stored and in a locked area or must be inaccessible and out of a childs reach. Provider advised that when she needs to dispense diaper cream, she will get parent authorization and keep it on file. This standard was brought back into compliance during the time of inspection.
