FDCH/LFCCH Handbook, Section 7.6, A (Section 2.1 Health and Safety, Page 3) Health Requirements [SR] Comments: During the time of inspection, Licensing Specialist observed on the screen in porch leading to the playground there is work out equipment in there.(The provider stated she is not going to use the entrance through the screen in porch to get to the playground no more and the door stays locked. The provider stated she will use the side gate on the side of the house to enter the play area.) On the play area there is gardening tools up against the house and there is weight lifting equipment by the fence on the left. On the play cars there is a green and black substance. The Child Care Attestation of Good Moral Character was not completed at the time of initial screening or upon change in employers. FDCH/LFCCH Handbook, Section 4.1, Rand4.3, B (Section 2.1 Health and Safety, Page 3) Staffing Requirements [SR] Comments: During the inspection, the Operator advised that there are currently 2 staff members and 1 household members. The Operator advised that 2 staff members and 1 household member that did not sign the new updated Good Moral Character form. The Operator and the household member signed the document at the time of the inspection. The Operator advised, the substitute will sign and send the document to the Licensing Specialist. The Operator has been given a due date of 11/10/25 to come into compliance with this standard. Child(ren) did not have a Student Health Examination/DH (Form 3040), or an equivalent health statement on file within 30 days of enrollment. FDCH/LFCCH Handbook, Section 8.2, A Children's Records [SR] Comments: During the inspection, Licensing Counselor monitored 5 of the 5 childrens records. Licensing Counselor found one child who had an expired health form. The provider brought this standard back in compliance at the time of inspection. During the inspection, Licensing Counselor provided technical assistance, FDCH/LFCCH Handbook, Section 8.1 Health/Immunization Records A. "The operator is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form, Parts A-1, B, and/or C, DH 680 (July 2010), or the Religious Exemption from Immunization form, DH 681 (July 2008), which are incorporated herein by reference in 65C-22.001(7)(o) and (p), F.A.C., from the custodial parent or legal guardian, within 30 days of enrollment. DH Form 680 and DH Form 681 may be obtained from the local health department. The parent/guardian of a child who has not received the age-appropriate immunizations prior to enrollment and who does not have documented medical or religious exemptions from routine childhood immunizations should provide documentation of a scheduled appointment or arrangement to receive immunizations. Providers must include a general statement in parent handbook/policies to inform parents/guardians, at time of enrollment, that some children in care may not have current immunizations. B. If the custodial parents or legal guardians fail to provide the above required documentation within 30 days of enrollment, the home shall not allow the child to remain in the program. Compliance Comments During the inspection, the owner advised that there are currently 5 children enrolled. During the inspection, Licensing Counselor monitored 5 of the 5 childrens records. REMINDER: Influenza brochures must be signed annually between August 1st and September 30th for each enrolled child.
