General Requirements [SR] Comments: 1:14 two year olds (7), three year olds (6) and four year old (1) 2:15 four year olds and five year olds CORRECTED RATIO 1:7 two year olds 1:7 three year olds and four year olds 2:15 four year olds and five year olds 3-1: During the inspection, Licensing Specialist observed that the three year old classroom was out of ratio, in that there was 1 teacher with 14 children (two year olds (7), three year olds (6) and a four year old (1)). During the inspection, the two year olds were brought to their classroom. This is being marked corrected at the time of the inspection. During the inspection, Licensing Specialist provided technical assistance, STAFF-TO-CHILDREN RATIO. (a) Minimum standards for the care of children in a licensed child care facility as established by rule of the department must include: 1. For children from birth through 1 year of age, there must be one child care personnel for every four children. 2. For children 1 year of age or older, but under 2 years of age, there must be one child care personnel for every six children. 3. For children 2 years of age or older, but under 3 years of age, there must be one child care personnel for every 11 children. 4. For children 3 years of age or older, but under 4 years of age, there must be one child care personnel for every 15 children. 5. For children 4 years of age or older, but under 5 years of age, there must be one child care personnel for every 20 children. 6. For children 5 years of age or older, there must be one child care personnel for every 25 children. 7. When children 2 years of age and older are in care, the staff-to-children ratio shall be based on the age group with the largest number of children within the group. The facilitys refrigerator and/or freezer does not have an appropriate thermometer. CCF Handbook, Section 3.9.2, F.1 Food and Nutrition [SR] Comments: During the inspection, the Operator advised that the facility provides meals and snacks to the children that are in care. ***The provider is a food program participant. **** 24-23: During the inspection, Licensing Specialist observed that the fridge in the VPK room did not have a thermometer. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 3.9.2, F.1 Refrigerators/freezers: 1. An accurate thermometer designed to measure cold storage temperature must be placed inside each refrigeration and freezer unit. Thermometers in refrigerators must show a reading of 41 degrees Fahrenheit or below, and thermometers in freezers must show a reading of 0 degrees Fahrenheit or below. The thermometer must be located in the center of the unit and be readily accessible. Thermometer temperature readings higher than specified above require further temperature testing of food samples stored in the unit using a probe type thermometer; and adjustments to the unit setting to reach and maintain the required readings must be made. This standard has been brought back into compliance in that the child was returned to the facility by an agency outside of the department. The ground cover or other protective surface under the [swing set] was not maintained. CCF Handbook, Section 3.12, D (Section 2.1 Health and Safety, Page 3) Sanitation and Equipment [SR] Comments: 32-7: Licensing Specialist observed that the facility did not have enough mulch to meet the 6 inch depth requirement around the swing set, in that the depth of pea gravel was 4 inches. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 3.12, D Permanent or stationary playground equipment must have a fall/use zone that extends a minimum of 6 feet in all directions from the perimeter of the equipment. All types of ground cover must be maintained to provide resilience and reduce the incidence of injuries to children in the event of falls. 1. If the ground cover in place is loose ground cover (such as, but not limited to: mulch, shredded rubber chips, or sand) a minimum of 6 inches in depth is required in the use zone. Asphalt, concrete, hard packed dirt, hay, grass or leaves are unsuitable for use in the use zone area. 2. If the ground cover in place is a unitary playground surface, then the unitary surfacing materials must be installed, maintained, or replaced according to manufacturers instructions. Unitary surfaces must be tested to and comply with ASTM F1292; documentation of test data must be retained at the facility and available for licensing to review. 3. If the play area was approved by the Department prior to January 1, 2020 and does not meet the 6-foot fall/use zone requirement, then the facility must submit a written notification, including photographs and layout of the play area, to the Department prior to or on April 1, 2020, notifying its intention to continue to operate using the prior approved play area. However, if the permanent or stationary playground equipment is moved or replaced then the standard would apply, and 6 feet of use zone must be provided. The placement of [new climber with slide] did not allow for adequate distance/clearance from other equipment, fences, and objects in the area. CCF Handbook, Section 3.12, C (Section 2.1 Health and Safety, Page 3) Sanitation and Equipment [SR] Comments: 32-8: During the inspection, Licensing Specialist observed that the facility had a new climber (with a slide) installed on the outdoor playground. Licensing Specialist observed that the new climber did not have the 6 foot clearance around the perimeter of the equipment, in that climber had a depth of six inches of pea gravel however, one side of the climber was 4 feet 1 inch away from a green plastic climbing dinosaur and another side of the climber was 4 feet 6 inches away from an orange plastic climbing dinosaur. The other two sides of the climber had a 6 feet clearance. During the inspection, Licensing Specialist provided technical assistance CCF Handbook, Section 3.12, D Permanent or stationary playground equipment must have a fall/use zone that extends a minimum of 6 feet in all directions from the perimeter of the equipment. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 7.1 The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form Part A-1, B, or C, DH 680, which is incorporated by reference in 65C-22.001(7)(o), F.A.C., or the Religious Exemption from Immunization form, DH 681, which is incorporated by reference in 65C- 22.001(7)(p), F.A.C., from the custodial parent or legal guardian. DH Form 680 and DH Form 681 may be obtained from the local county health department. Specific immunization requirements are included and detailed in the most current edition of the Immunization Guidelines-Florida Schools, Child Care Facilities and Family Day Care Homes as promulgated by the Florida Department of Health. 42.
