All child care personnel who completed DCF training did not complete additional DEL-approved health and safety training by required date. (Section 3.2.A.1. and 3.2.C Training Requirements, Page 7) [SR] Comments: During the inspection, the Licensing Specialist observed one staff member who had completed the DCF training did not complete additional DEL School Readiness trainings. The facility was given 14 days to complete the requirement per the School Readiness handbook. The Licensing Specialist provided technical assistance from the School Readiness handbook Section 3.2.Aa and 3.2.C 2. Licensed School Readiness providers and any other SR provider type may complete: Option 1 (DCF courses, plus corresponding exams) Existing courses available now under the Child Care Facility Part 1 heading for $10 per course: Health, Safety and Nutrition (8 hours) Child Growth and Development (6 hours) Behavioral Observation and Screening (6 hours) Identifying and Reporting Child Abuse and Neglect (4 hours) PLUS Health and Nutrition in the School Readiness Program (4 hours) Safety Practices in the School Readiness Program (5 hours) 3.2 C. C. As part of the required health and safety training, all School Readiness child care personnel must complete training in pediatric First Aid and Cardiopulmonary Resuscitation by the timeframes established in section 3, Training Requirements, of this Handbook. Child(ren) did not have a Student Health Examination/DH (Form 3040), or an equivalent health statement on file within 30 days of enrollment. CCF Handbook, Section 7.2, C Record Keeping [SR] Comments: At time of the inspection, the Licensing Specialist observed one child's file did not have a current Student Health Examination within 30 days of Enrollment. The Licensing Specialist provided technical assistance from the handbook. Section 7.2, 7.2 Student Health Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Student Health Examination form DH 3040, which is incorporated by reference in 65C-22.001(7)(q), F.A.C. and may be obtained from the local county health department, the parent or legal guardian, or a signed statement by authorized professionals that indicate the results of the components of the Student Health Examination form are included in the health examination. A. The Student Health Examination shall be completed by a person given statutory authority to perform health examinations. B. The Student Health Examination or the signed statement is valid for two years from the date the physical was performed. An up-to-date version must be on file for as long as the child is enrolled at the facility. C. If the custodial parents or legal guardians fail to provide the documentation required above within 30 days of enrollment, the facility shall not allow the child to remain in the program. Compliance Comments During the time of the inspection, the Director advised the Licensing Specialist that there are currently 87 children enrolled, 18 of which had been newly enrolled since the last inspection. 04. Supervision CCF Handbook, Section 2.4 (04-01)

Supervision of children in the [Age 2 ] group was inadequate in that [a teacher left the classroom without verifing the other teacher was present]. CCF Handbook, Section 2.4 General Requirements [SR] Comments: The director reported that one of the staff left the classroom to fill her water cup after motioning to the second staff in the room. When the staff returned to the classroom a few minutes later, the second teacher was not present. The issue was resolved promptly as the staff was in the classroom and supervising the two-year-old children in the classroom. The Director advised the staff was spoken to at the facility about the supervision of students. The Directors will continue to monitor teachers, staff, and provide trainings on proper supervision and communication within the classroom within hours of operation It is noted the facility has a zero-tolerance policy. The Licensing Specialist (LS) provided technical assistance from the handbook Section 2.42.4.1 General Supervision Requirements A. Child care personnel must position themselves in the outdoor play area so that all children can be observed and directly supervised. B. Child care personnel must be assigned to provide direct supervision to a specific group of children and be with that group of children at all times. Children must never be left without child care personnel supervision inside or outside the facility, in a vehicle, or at a field trip location. C. Supervision standards apply at all times away from the child care facility, including during field trips, outdoor play, and when picking up or dropping off children at designated locations, such as bus stops, school, or a childs home. For school-age children, personnel must know where the children are and what they are doing at all times and capable of responding to emergencies including when children are separated from their group. D. At all times, lighting must allow child care personnel to see and supervise children while in care. E. A program is responsible for the supervision of a child until an authorized individual retrieves the child from the program. A child shall not be released to any person other than the person(s) authorized or in the manner authorized in writing by the custodial parent or legal guardians. All individuals authorized to pick up a child must be identified in writing prior to release by the custodial parent or legal guardian to the program, and the program must verify the individual picking up the child is authorized by using a picture form of identification. Each child transported must be dropped at the designated location as agreed upon by the provider and the custodial parent/legal guardian and released to an authorized individual. F. No person shall be an operator, owner, or employee of a child care facility while using or under the influence of narcotics, alcohol, or other drugs that impair a persons ability to provide safe care and supervision. G. In addition to the number of child care personnel required to meet the staff-to child ratio, one additional adult must attend field trips away from the child care facility to help provide direct supervision. The individual could be a parent volunteer as long as that person is under direct and constant supervision of a screened and trained child care personnel. H. Children must receive supervision and care in accordance with their age and required needs and be accounted for at all times while bathing or toileting. I. When transporting children in a vehicle or on foot, a telephone or other means of instant communication must be available to child care personnel. The facility did not have documentation to show completion of a Department approved five-hour early literacy and language development course for child care personnel within 12 months of date of employment in child care industry and/or the early literacy course documentation was not uploaded in the Florida Pathways/Registry. CCF Handbook, Section 4.2.2 and 4.5, C (Section 2.1 Health and Safety, Page 3) Comments: At the time of the inspection, the counselor observed two staff record did not have documentation of completing Literacy training within 12 months of the date of employment in the child care industry. Provided technical assistance from the handbook Section 4.2.2 Early Literacy Training Pursuant to Section 402.305(2)(e)5., F.S., all child care personnel must complete a single course of training in early literacy and language development of children ages birth through five years that is a minimum of five clock hours or .5 CEUs. Early literacy training must be completed within 12 months of date of employment in the child care industry. Proof of completion may be documented on a certificate of course completion, classroom transcript, or diploma. Child care personnel must complete one of the following: A. One of the Departments online literacy courses available on the Departments website. B. One of the Departments approved literacy courses.
