   Sec. 7-33(e), Ordinance: There shall be at least one child care staff member providing direct supervision at all times when one or more children are present. At no time shall any children be left alone for any reason without direct supervision by a child care staff member. Direct supervision means watching and directing the childrens activities within close proximity in the same room or within a designated outdoor play area, and responding to each childs needs. Child care personnel and volunteers at a facility must be assigned to provide care to a specific group of children and be present with that group at all times during the day, including during meals, napping, and transportation of children 2.4.1(B), Handbook: Children must never be left inside or outside the facility, in a vehicle, or at a field trip location by themselves. Lack of direct supervision occurred when [the teacher left the room unsupervised to collect some toys outside the classroom. ]. Ratios/Discipline [SR] Comments: Complaint alleges that on 10/6/2025,at pickup time, the parent noticed that the child's pinky finger nail plate was missing. His whole finger squeezed off. The parent brought it to the teacher Ms. T attention and she was told that the child was playing with someone and was pushed. Thee cameras were checked and the teacher left the classroom leaving the children unattended and running around in the classroom. Someone pushed the door open and his finger was squeezed between the door jamb and the child was screaming. FINDINGS: A complaint inspection was conducted on 10/8/2025 and a statement from the owner and teacher was received. Per the teacher N. Williams, she had left the room to collect her classroom toys that were drying up under the cubbies in the 3y/o class. She asked the 3y/o teacher to keep an eye on her children but she was also keeping an eye on them. Then she saw the child walking toward her with blood in his mouth and on his finger. She assumed he bit his finger because of the blood she saw in his mouth. She called the director, and told her that the child may have bit his finger. She proceeded to place the finger under running water, then dressed the finger with gauze. A band-aid was then applied on the child's finger. She believes she was out fo the room for no more than five minutes when the incident happened. Per the Owner T. Spence, she was not onsite when the incident occurred. She received a call from the office staff member who informed her that the child bit his finger. She then called the parent twice to relay the information but the parent didn't answer. She then texted the parent to tell her that she had something important to tell her. The parent called back and was informed that the child bit his finger. She was checking the cameras as she was on her way to the school. That is when she noticed that the child did not bite his finger but got his finger jammed in the door. Upon arrival, the parent removed the band aid and saw that the severity of the injury. The owner had also not seen that the child's nail came off until the parent removed the Band-Aid. Video footage reviewed and the following was observed; The children were observed standing by the classroom entrance door and the door was slightly ajar. One of the children was holding the door knob and she closed the door. When the child closed the door DS's fingers got caught in between the threshold. The child quickly pulled his finger out and held it with his other hand, crying. Five children were observed unsupervised at the time of the incident. This complaint is substantiated for a lack of direct supervision and a class 1 non-compliance and Notice of Violation is being issued. 08.
