4.2.4(B), Handbook: the facility shall maintain the following number of child care personnel with the CPR certification and first aid training that are on site at the facility at all times children are in care. For child care personnel to count to meet requirement in the below chart, they shall be child care personnel assigned to provide direct supervision of children in care: 1. For 0-25 child care personnel in direct supervision of children to meet staff to child operating ratios there must be 3 child care personnel with pediatric CPR and first aid certification, 2. For 26-50 child care personnel in direct supervision of children to meet staff to child operating ratios there must be 4 child care personnel with pediatric CPR and first aid certification, 3. For 51-75 child care personnel in direct supervision of children to meet staff to child operating ratios there must be 5 child care personnel with pediatric CPR and first aid certification, 4. For 76-100 child care personnel in direct supervision of children to meet staff to child operating ratios there must be 6 child care personnel with pediatric CPR and first aid certification, 5. For 101+ child care personnel in direct supervision of children to meet staff to child operating ratios there must be 7 child care personnel with pediatric CPR and first aid certification The facility had an inadequate number of child care personnel appropriately trained in pediatric CPR and/or first aid. Sec. 7-43(a), Ordinance: Within 15 days after enrollment, the child care facility must obtain, from a parent for each child in care, a current, complete, and properly executed Florida Certification of Immunization form Part A-1, B, or C, DH 680, which is incorporated by reference in Rule 65C-22.001(7)(o), Florida Administrative Code, or the Religious Exemption from Immunization form, DH 681, which is incorporated by reference in Rule 65C-22.001(7)(p), Florida Administrative Code. DH Form 680 and DH Form 681 may be obtained from the Florida Department of Health. The Florida Certificate of immunization was missing for children in care. Sec. 7-45(b)(2), Ordinance: All trash, debris, and dirt must be removed from the vehicles daily. All seats, safety restraint devices, and fixtures must be maintained free of rips and tears and in good repair at all times. Vehicles must be cleaned and sanitized routinely. The facility failed to maintain the vehicle as evidenced by [See below]. Comments: One of the seat belts was missing a buckle and therefore is inoperable. Sec. 7-45(b)(1), Ordinance: Vehicles must state the name of the child care facility on each side panel in six- to eight-inch letters. The back panel must have the wording Caution Transporting Children in a minimum of four-inch letters. The facilitys license number also must be printed on the side and back panels of the vehicle. The facility's vehicle(s) did not meet the requirement in that [see below]. Comments: The vehicle was missing the signage on one side, and the other side did not meet measurement requirements. 44. 5.1(A), Handbook: A screening must be conducted as a condition of employment. Level 2 screening as outlined in s. 435.04, F.S., is required for all child care personnel and includes a criminal records check (both national and statewide), a sexual predator and sexual offender registry search, and child abuse and neglect history of any state in which an individual resided during the preceding 5 years. Documentation of Level 2 Clearinghouse screening clearance was missing for child care personnel. Staff Requirements [SR] Comments: Two staff members files were missing proof of background screening. 4.2.6, Handbook: All child care personnel participating in the transportation of children must have training regarding guidance on safe transportation practices prior to participating in transportation of children. For child care personnel to satisfy this requirement the training must be accomplished through the Departments Transportation Safety course or the Early Learning Floridas Precautions in Transporting Children course. Documentation of training must be maintained on the training transcript in the child care personnel file. The facility did not have documented proof that child care personnel participating in the transportation of children were trained on safe transportation practices prior to participating in transportation of children. [SR] Comments: There was no proof of transportation training for staff member participating in transportation.
