3.10.2(J) and (K), Handbook: Soiled or wet disposable diapers must be disposed of in a plastic lined, securely covered container that is not accessible to the children. The container must be emptied, cleaned and sanitized or disinfected, at least, daily. Soiled cloth diapers must be emptied of feces in the toilet and soiled or wet cloth diapers shall be placed in a securely covered container that is not accessible to the children. The container must be emptied, cleaned and sanitized or disinfected, at least, daily. The storage of soiled diapers/clothing was not sanitary. Toilet/Bath Facility [SR] Comments: Complaint alleges that the child C.T. (DOB: 04/06/2023) began attending the provider on Monday 4/20/26 and when she picked him up his pull up was leaking urine because it was never changed. FINDINGS: A complaint inspection was conducted on 4/28/2026 and video footage of the day was reviewed. The child was observed being changed on two different occasions. First, upon arrival at 12:17PM. Second time was prior to snack time at 3:37PM. According to the schedule, routine potty training should have occurred two more times between 4:30PM - 5:00PM and between 5:15PM -5:30PM. This complaint is partly substantiated as the child was not changed/checked after 3:37PM. He was changed twice while in care. 26. Sec. 7-33(e), Ordinance: Direct supervision means watching and directing the childrens activities within close proximity within the same room inside or within a designated outdoor play area and responding to each childs needs. Inadequate supervision occurred when a child/childrens needs were not met in the following incident: [A child broke a glass bowl and injured another child]. Ratios/Discipline [SR] Comments: Complaint alleges that on 05/09/2025, the head teacher was going to do a science project with the kids and left a glass item on the ground while the kids were sitting on the carpet. An ESE student proceeded to stomp on the glass item, causing it to shatter, and the shattered glass cut multiple students. The teachers immediately took the students outside to treat the cuts. Findings: A complaint inspection was conducted on 7/3/2025 and a statement from the teacher was received. Per the teacher, she left the glass item on a low table and went to get water for a sink and float science project. There was an assistant teacher in the class with the children when the lead teacher walked out. That is when child A stomped on the glass, shattering it and hurting a few students. She didn't see the incident, she just saw the blood when she returned to the classroom. She tended to the children by washing the cut and applying a band aid. She believes it was about 2 or 3 children that had minor cuts. Per the director, the only child that was hurt was child B. There were no other children that were hurt. After the incident, all glass items used for children activities were removed. Complaint is substantiated. Source: FL DCF CARES inspection 2025-07-03. Sec. 7-5.01(l)(1), Ordinance: Drinking water shall be available and accessible to the children at all times from a public water system or other water source approved and regulated by the State of Florida. Drinking water means potable water safe for human consumption. Use of a garden hose to provide drinking water is prohibited due to potential health risks that could result from cross connections and backflow contamination of the water system. The drinking fountain was in need of repair. Facility/Storage [SR] Comments: Inside water fountain in the hallway is malfunctioning because the handle does not turn appropriately , the children can not operate it.
