Sec. 7-38(b)(4) and (5), Ordinance: Infants shall not be placed for napping or sleeping in any car safety seat, bean bag chair, bouncy seat, infant seat, swing, jumping chair, highchair, chair, futon, sofa, or any other type of furniture or equipment that does not meet the State Requirements for cribs and play yards. Infants who arrive at the facility asleep in a car safety seat must be immediately removed from the seat by the parent or the child care personnel and placed on their backs in a safe sleep environment. An infant was observed sleeping outside of a crib/play yard. Indoor/Outdoor Space [SR] Comments: During re-visit inspection, an infant was observed napping in an infant swing, with a blanket over her face and with a propped bottle. Monitor asked the teacher to place the child in the crib/play yard. The child was moved to the play yard but woke up from sleeping. The teacher then rocked the child, child remained awake. Each fire drill record must be maintained for a minimum of 12 months from the date of the fire drill. The facilitys record of fire drills was missing [proof of fire drills from July to October. ], or was not maintained for 12 months. Sec. 7-44(f), Ordinance: The childs arrival and departure must be recorded by child care personnel at the time the child enters and departs the child care facility. The attendance sheet/class roster must accompany the child care personnel and the group of children throughout the day should they leave the classroom. In order to account for childrens whereabouts, children who are reassigned to different classrooms throughout the day should be signed in and/or out of the rosters for those respective classrooms each time. The facility failed to maintain an accurate attendance roster for the children in care. (Section 2.1(A) Health and Safety, Page 3) License/Certificates [SR] Comments: The mixed group of 3s, 4s, and aftercare attendance was checked for compliance. 3.12(C), Handbook: All equipment, fences, and objects on the facilitys premises shall be free from sharp, broken and jagged edges, and properly placed to prevent overcrowding or safety hazards in any one area. Equipment in the outdoor space needs repair. Indoor/Outdoor Space [SR] Comments: The outdoor play structure had cracks and tear in the middle of the red slide causing a small gap. The 2y/o class was in the playground and 13 children were observed with 1 staff member. The toddler class was in the classroom and 8 children were observed with 1 staff member.
