3.12(D)(1) and (2), Handbook: Permanent or stationary playground equipment must have a fall/use zone that extends a minimum of 6 feet in all directions from the perimeter of the equipment. If the ground cover in place is loose ground cover (such as, but not limited to: mulch, shredded rubber chips, or sand) a minimum of 6 inches in depth is required in the use zone. Asphalt, concrete, hard packed dirt, hay, grass or leaves are unsuitable for use in the use zone area. If the ground cover in place is a unitary playground surface, then the unitary surfacing materials must be installed, maintained, or replaced according to manufacturers instructions. Unitary surfaces must be tested to and comply with ASTM F1292; documentation of test data must be retained at the facility and available for licensing to review. 3.1(A) and (G), Handbook: All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation.Cleaning must not take place while rooms are occupied by children except for general clean-up activities that are part of the daily routine.This does not include cleaning with hazardous materials or any cleaning which poses a risk of slipping or falling.Areas of the facility were found to not be clean, sanitary or well maintained Facility/Storage [SR] Comments: The following areas were found in need of repair: - Kitchen floor laminate was observed lifting up and cracked. - Some toys in the playground bin were observed broken. - Mulch need to be added to the playground surface. - Garbage bins need to be cleaned. - Hose at the playground needs to be stored properly. - Playground needs to be clean and free of trash. - Torn green mat around tree by the basketball court needs to be fixed or removed. - 3 seat belts faster were observed cracked, and a middle seat with 3 holes. Allow time for food to cool before serving to children that does not exceed 15 minutes.Food was served at an improper temperature.
