The health assessment statement shall be documented on a form prescribed by the local licensing agency and updated at least every two years. 5.1(A), Handbook: A screening must be conducted as a condition of employment. Level 2 screening as outlined in s. 435.04, F.S., is required for all child care personnel and includes a criminal records check (both national and statewide), a sexual predator and sexual offender registry search, and child abuse and neglect history of any state in which an individual resided during the preceding 5 years. Documentation of Level 2 Clearinghouse screening clearance was missing for child care personnel. Comments: There was no documentation available of a completed background screening for one of the childcare staff members. The staff member was observed working in the classroom alongside other employees who have screening clearance. 5.1(K), Handbook: The employer/owner/operator must add child care personnel to their Employee/Contractor Roster in the Clearinghouse when the individual has received a child care eligible result and has been hired at the facility. Employer/owner/operator must add an end date for individuals on the Employee/Contractor Roster in the Clearinghouse within 10 days of the employment termination.The Clearinghouse roster was not updated. Comments: The clearinghouse roster was not updated for a staff member who serves as a campus monitor. The staff member has an eligible school board of Broward County screening but requires agency review for DCF childcare eligibility. Provider stated that the campus monitor is employed by child care.
