These items must be cleaned and sanitized or disinfected immediately or prior to another childs use if exposed to bodily fluids, such as saliva. Toys/equipment were not sanitary in that [infants observed mouthing toys and toys not sanitized]. Indoor/Outdoor Space [SR] Comments: Complaint alleges that there are unsanitary toy and equipment sharing: Infants and toddlers share toys, playpens, and other items throughout the day without proper cleaning or sanitization between uses. FINDINGS: A complaint inspection was conducted on 10/21/2025 and a walkthrough of the infant room was conducted. The infants were observed playing with and mouthing toys. The teacher were observed putting toys away in the mat play area without separating the mouthed toys for sanitizing. This complaint is substantiated. 18.    3.6.1(I) and 3.6.3, Handbook: Children up to one year of age must be in a crib or playpen/play yard with sides for napping. No double or multi-deck cribs, cots, or beds may be used. 7-38(b)(6), Ordinance: Only one infant at a time shall be in any crib or play yard for napping or sleeping. The facility did not have enough cribs or playpens with sides for the number of infants in care. Indoor/Outdoor Space [SR] Comments: Complaint alleges that the school's Playpen sheets are often not changed between children, even when visibly soiled. FINDINGS: A complaint inspection was conducted on 10/21/2025 and the following was observed; Four playpens and five infants were observed. Per director, there are 5 playpens, 4 in the classroom and one is kept in the hallway. After my walkthrough i did not see the 5th playpen. Then, the director was observed going to a vehicle and retrieving the 5th playpen. The director stated that she takes them home to clean over the weekend.    3.6.4(B), Handbook: When napping or sleeping, young infants who are not able to roll over must be positioned on their backs and on a firm surface to reduce the risk of Sudden Infant Death Syndrome, unless an alternate position is authorized by a physician. Sleep sacks that fit according to manufacturers recommendations, do not restrict the infants arms, and will not slide up around the infants face may be used for the comfort of the sleeping infant; however, swaddling shall not be used unless authorized in writing by the childs physician. Written documentation of a physicians authorization must be on file at the facility. Documentation must include the childs name, childs date of birth, description of sleep position required, instructions for the use of any equipment needed, and length of time authorization is valid. There was no documentation on file that an alternative position/swaddling was authorized by a physician for an infant in care. Indoor/Outdoor Space [SR] Comments: Complaint alleges that the school has Unsafe sleep practices: parent repeatedly witnessed infants sleeping in swing chairs, and other children sleeping with blankets placed over their heads. FINDINGS: A complaint inspection was conducted on 10/21/2025 and a statement from the director was received. Per the director, there is a child that may sleep on a pillow or swing because the child has a health condition that required her to have an alternate sleep position. No other children sleep in an alternate position. The child's file was checked but there was no documentation on file that an alternative position/swaddling was authorized by a physician for an infant in care. TA was provided that written documentation of a physicians authorization must be on file at the facility. Toilet/Bath Facility [SR] Comments: Complaint alleges that the facility fails to use gloves: Staff frequently do not wear gloves when changing diapers, wiping noses, or handling food, and often reuse the same gloves for multiple tasks. FINDINGS: A complaint inspection was conducted on 10/21/2025 and the following was observed; The teacher (CL) was observed changing diapers during today's inspection. The teacher was observed to have changed two diapers without changing gloves. The diaper changing pad was not disinfected after use. This complaint is partly substantiated. 26.
