Sec. 7-33(e), Ordinance: Direct supervision means watching and directing the childrens activities within close proximity within the same room inside or within a designated outdoor play area and responding to each childs needs. Inadequate supervision occurred when a child/childrens needs were not met in the following incident: [The 3-year-old classroom with a group of 9 napping children was without supervision at the time of my arrival.]. Ratios/Discipline [SR] Comments: The 3-year-old classroom with a group of 9 napping children was without supervision at the time of my arrival. The staff member M. V. responsible for the 3-year-old classroom was observed in the VPK classroom. Upon seeing me, the staff member walked from the VPK classroom to the 3-year-old classroom. When asked where she was at the time of my arrival and who was covering her classroom because she was observed in VPK, the staff member stated that another staff member. J.C. was in the room. When asked where he was now, she stated that he was in the bathroom. A few minutes later, the director M.M. appeared. I asked her where she was when I arrived and she said that she had been in the bathroom. I asked her where J.C. was and she stated that he was doing the bus run. I went back to the 3-year-old classroom and asked M.V again who had been in her classroom when I arrived, she then stated that the staff member responsible for the 2-year-old classroom (S.M.) had been standing in between the two classrooms while she was in the VPK classroom. The corporate manager, N.C., arrived and I explained to him what was observed. He then asked the staff member M. V. where she had been when I arrived. She changed her story again, saying that she had been in the VPK classroom because she needed to sign a document and the staff member responsible for the VPK classroom (D.Q.) had been in her classroom (the 3s), and then they switched back to their respective rooms. When I arrived, D.Q. was the first staff member I encountered by the door used to enter the VPK classroom and was not observed in the 3-year-old classroom. J.C. was not present at the facility at the time of my arrival; he arrived approximately 20 minutes later. The following ratios were observed shortly before departure: -1's- 1:6 -2's- 1:9 -3's 1:9 -VPK- 1:10 -schoolage- 2:14 This inspection report was updated and changed to the standard of Inadequate Supervision-Class II. The crib/play yard sheets were not appropriate. Indoor/Outdoor Space [SR] Comments: 1 crib in the infant room had a sheet that was loose and not tightly fitted. The sheet was changed during the visit. Following progressive enforcement procedures, a Notice of Violation is being issued as this standard was previously cited out of compliance on 7/22/25. 21. 3.2(A), Handbook: All areas and surfaces accessible to children must be free from toxic substances, bio contaminants, and hazardous materials/equipment/tools, including power tools, plastic bags, matches, candles, lighters, etc. A toxic/hazardous material was accessible to children. Facility/Storage [SR] Comments: Cans of paint were observed along the route in the patio area used by the children when conducting fire/evacuation drills at the time of this inspection. The paint must be stored in an area that is inaccessible to the children at all times. The provider was given a verbal technical assistance regarding this area during the inspection on 3/4/25. A refrigerator was observed with a thermometer showing a temperature reading greater than 41 degrees Fahrenheit and/or freezer was observed with a thermometer showing a temperature reading greater than 0 degrees Fahrenheit. Food Service/Nutrition [SR] Comments: 3 thermometers in the refrigerator used to house the cold foods, milk, etc. for the children were observed to be reading between 46 and 50 degrees Fahrenheit at the time of this inspection. Sec. 7-45(b)(2), Ordinance: All trash, debris, and dirt must be removed from the vehicles daily. All seats, safety restraint devices, and fixtures must be maintained free of rips and tears and in good repair at all times. Vehicles must be cleaned and sanitized routinely. The facility failed to maintain the vehicle as evidenced by [SEE COMMENTS]. Comments: The plastic cover for the side of the 1st seat on the left is missing and needs to be replaced/repaired. TECHNICAL ASSISTANCE: Sec. 7-45(b)(1), Ordinance: Vehicles must state the name of the child care facility on each side panel in six- to eight-inch letters. The back panel must have the wording Caution Transporting Children in a minimum of four-inch letters. The facilitys license number also must be printed on the side and back panels of the vehicle. 3.6.3, Handbook: Cribs or playpen/play yards must meet the regulations as outlined in Title 16, Parts 1219, 1220 & 1221 Code of Federal Regulations, 2014, which is incorporated by reference in 65C-22.001(7)(w), F.A.C. The facility was using cribs/play yards that did not meet federal guidelines. (Section 2.1(A) Health and Safety, Page 3) Indoor/Outdoor Space [SR] Comments: During the walkthrough of the facility, a child was observed napping in a crib in the infant room that did not have a certificate of compliance available for review that indicated the crib's date of manufacture, as required. It could not be verified that the crib meets current federal regulations. This area was brought into compliance during the inspection when the crib was removed from the classroom. The director and owner have been made aware that the crib cannot be used until a certificate of compliance can be obtained and verified by the licensing monitor. Compliance Comments TECHNICAL ASSISTANCE: Section 3.5(F), Handbook: The outdoor play area must have adequate fencing or walls a minimum of 4 feet in height.
