   Sec. 402.305(4)(a), Florida Statutes: Minimum standards for the care of children in a licensed child care facility as established by rule of the department must include: 1. For children from birth through 1 year of age, there must be one child care personnel for every four children. 2. For children 1 year of age or older, but under 2 years of age, there must be one child care personnel for every six children. 3. For children 2 years of age or older, but under 3 years of age, there must be one child care personnel for every 11 children. 4. For children 3 years of age or older, but under 4 years of age, there must be one child care personnel for every 15 children. 5. For children 4 years of age or older, but under 5 years of age, there must be one child care personnel for every 20 children. 6. For children 5 years of age or older, there must be one child care personnel for every 25 children. A ratio of [1] child care personnel for [25] children is required. A ratio of [1] child care personnel for [27] children was observed. Ratios/Discipline [SR] Comments: Complaint received by this inspector on 1/21/25. It was investigated on 1/22/25. It stated the following: The parent of MR got a phone call from the school stating that the child fell on the playground. The complaint further alleges that there was blood pouring from 3 front teeth, upper lip was busted and teeth were broken.    Sec. 7-33(e), Ordinance: Direct supervision means watching and directing the childrens activities within close proximity within the same room inside or within a designated outdoor play area and responding to each childs needs. Inadequate supervision occurred when a child/childrens needs were not met in the following incident: [see narrative]. Ratios/Discipline [SR] Comments: Complaint received by this inspector on 1/21/25. It was investigated on 1/22/25. It stated the following: The parent of MR got a phone call from the school stating that the child fell on the playground. The complaint further alleges that there was blood pouring from 3 front teeth, upper lip was busted and teeth were broken. The staff present during this incident was interviewed. She indicated that she was sitting at a picnic bench on the playground facing the basketball court. She further stated that she did not see the incident occur as she was talking to a child who was standing in front of her. MR approached the staff member. Another child witnessed the incident and told the staff member what happened. Based on interview and the incident report written by staff and signed by the parent, ice was applied and the area cleaned with water. This portion of the complaint is substantiated as the teacher did not position herself in a manner that allowed for adequate supervision. 08.
