Sec. 7-35(c)(2), Ordinance: Any child care facility with a licensed capacity of 50 children or fewer shall install a minimum of two drinking fountains, including at least one inside the facility and at least one in any outdoor play area. Child care facilities with a licensed capacity of more than 50 children shall have one additional drinking fountain located either inside or outside the facility for each additional 50 children licensed capacity or any fraction thereof. Manual or electrically powered commercial water coolers may be used to meet the indoor drinking fountain requirements. Sec. 7-33(e), Ordinance: There shall be at least one child care staff member providing direct supervision at all times when one or more children are present. At no time shall any children be left alone for any reason without direct supervision by a child care staff member. Direct supervision means watching and directing the childrens activities within close proximity in the same room or within a designated outdoor play area, and responding to each childs needs. Child care personnel and volunteers at a facility must be assigned to provide care to a specific group of children and be present with that group at all times during the day, including during meals, napping, and transportation of children 2.4.1(B), Handbook: Children must never be left inside or outside the facility, in a vehicle, or at a field trip location by themselves. Lack of direct supervision was observed in the [1y/o classroom and field trip to swim school]. Ratios/Discipline [SR] Comments: Allegation: Complaint alleges that the preschool is understaffed. There are times that a staff member will be responsible for 30 children at a time and take them outside. At times those staff members will leave the children outside, stepping inside for a minute or two, leaving the children unsupervised. If a teacher has to use the restroom, the children are left unsupervised. FINDINGS: During onsite investigation on 4/30/2025, A lack of direct supervision occurred on two different instances. Upon arrival, Teacher A was observed walking into the classroom with four 1y/o children. The teacher led the children into the 1y/o classroom and proceeded to open the door for me, leaving the children unattended in the room. Then, teacher walked down the hallway toward the VPK room. I stopped her and asked her if she is an adult required for ratio in her class and she said yes. I told her to go back into her class and I would go to the VPK room to speak to the VPK teacher. A walkthrough of the classrooms was conducted and staff communicated to me that some of the children were in swim class at the swim school located next door. At around 11:30AM, 10 children from ages 2 to 4 were observed walked back to the school with 3 adults. One of the three adults was teacher B, an employee of the preschool. The other two adults were employees of the swim school. After getting further information from Teacher B, it was determined that the children are being picked up by swimming school staff at around 9:49am, signed out in a log sheet and walked to the swim school. Teacher B stated that she walks with them to the swim school to change the children's clothes and then walks back to the school leaving the children with the swimming instructors for the duration of the lesson (1 hour - 10:00AM to 11:00AM). At 11:00AM, she walks to the swim school, changes the children's clothes, and walks them back to the school. She stated that swim lessons are held on Wednesdays for the duration of one hour. Complaint is substantiated for a lack of supervision. A class 1 lack of direct supervision and notice of violation is being issued. 08. The date, time, and location of the field trip must be posted in an easily seen location at least two working days prior to each field trip. Written parental permission must be obtained in the form of a general or event-specific permission slip. Parental permission was not obtained prior to field trip activity. 2.5.2(A), Handbook: A log must be maintained for all children being transported in a vehicle or on foot away from and/or to the premises of the child care facility. The facility did not have evidence that a log was maintained for children transported. Transportation [SR] Comments: During the course of a complaint inspection, the following was observed; A walkthrough of the classrooms was conducted and staff communicated to me that some of the children were in swim class, at the school school located next door. After getting further information from staff, it was determined that the children are being picked up by swimming school staff and walked to the swim school. Teacher A and two swim school staff were observed walking the children back to the school at around 11:26AM. A swim school staff member signed a sign in and out log located in the front desk of the school. TA was provided that a transportation log must be used when transporting children weather in vehicle or by foot. The transportation log must accompany the children on the field trip. Sec. 7-44(f), Ordinance: The childs arrival and departure must be recorded by child care personnel at the time the child enters and departs the child care facility. The attendance sheet/class roster must accompany the child care personnel and the group of children throughout the day should they leave the classroom. In order to account for childrens whereabouts, children who are reassigned to different classrooms throughout the day should be signed in and/or out of the rosters for those respective classrooms each time. The facility failed to maintain an attendance roster for the children in care. License/Certificates [SR] Comments: During a complaint inspection related to supervision, the following non-compliance was observed; None of the four classrooms occupied by children had an attendance sheet/ classroom roster. The attendance sheet/class roster must accompany the child care personnel and the group of children throughout the day should they leave the classroom. 3.12(C), Handbook: All equipment, fences, and objects on the facilitys premises shall be free from sharp, broken and jagged edges, and properly placed to prevent overcrowding or safety hazards in any one area. Equipment in the outdoor space needs repair. (Section 2.1(A) Health and Safety, Page 3) Indoor/Outdoor Space [SR] Comments: At the time of this inspection the following concerns were noted on the outdoor play space: clean yellow climbing stairs on large play piece remove rust from the blue rings and repaint the rings repaint the hinges on the large red and blue play piece repaint the red pipes on the piece near the fence and ensure that the pipe is secured to the piece.
