Sec. 7-33(e), Ordinance: Direct supervision means watching and directing the childrens activities within close proximity within the same room inside or within a designated outdoor play area and responding to each childs needs. Inadequate supervision occurred when a child/childrens needs were not met in the following incident: [child lost shoe lace at school, not diapered frequently, scratch on back]. Ratios/Discipline [SR] Comments: Complaint alleges that child has repeatedly been left in soiled diapers, including instances where urine pampers have been left on him, and on several occasions, parent has picked him up with feces still in his diaper. When parent enquires about the last time he was changed, she is told it was after nap time (around 1:00 p.m.), and parent does not pick him up until after 5:00 p.m. Lost Shoe Laces: Child's shoe laces have gone missing, and when parent asked about it, she was told, Not sure what happened, mommy. This situation presents a hazard, as there is a risk that my child may trip and fall, or worse, cause another child to trip over loose laces. Unexplained Injury: On December 10th, parent noticed a deep scratch in the middle of his back. When she asked the staff about the injury, she was again told, We dont know, mommy. Lastly, I am extremely concerned about the lack of communication from the school. A complaint inspection was conducted on 12/20/2024 and a statement from the director and teachers was received. Per the director(Sandra ), the parent did mention that the child's shoelace was missing at pick up time to the teacher that was caring for him. The teacher said she received him like that from his assigned class. The director said that she and the parent went to the child's assigned classroom to look for it but didn't find it. The staff so not know what happened to his shoelace. Per the assigned teacher(Betty), she had already left for the day when the parent picked up so she doesn't know anything about the shoelace. The teacher does not know how the child received the scratch on his back. She saw the scratch the following day and said apologized to the parent but didn't know how he received it. She stated that she is more vigilant of him now because he is the youngest child in the class. The teacher was asked about the diapering times and she stated that he is the only one in diapers. All the other children are already potty trained. Per his previous teacher (2y/o class ), she stated that the mom has expressed that he isn't being changed for long periods of time so she said she tries to go in the 3 year old room to change him before she leaves for the day. This complaint is being substantiated for inadequate supervision. In both incidents regarding the scratch and the missing shoelace, the staff are unaware of what happened to the child. The child has been left overly soiled so another teacher from a different class has had to tend to him.
