44. Drivers Qualifications/Requirements Group: Sec. 7-45, Ordinance and DCF Handbook (44-06)

2.5.2(A)(1-5), Handbook: A log must be maintained for all children being transported in a vehicle or on foot away from and/or to the premises of the child care facility. The log must include: (1.) Each childs name, (2.) The date and time of departure, (3.) Time of arrival at the destination, (4.) The signature of the driver (or in the case of travelling on foot, the signature of the child care personnel), and (5.) The signature of a second child care personnel or person(s) authorized by the provider to verify the transportation log and that all children have arrived safely and left the vehicle (if applicable). The facilitys log did not include all required information. Comments: The signature of a second child care personnel or person authorized by the provider to verify the transportation log was not recorded for April 1, 2026, and April 2, 2026. Provider needs to submit via email a copy of the documented transportation log daily on the following dates: Apri 7, 2026, April 8, 2026, and April 9, 2026, to verify this area is in compliance with transportation log requirements. 10. General Facility Requirements Group: Sec. 7-35, Ordinance and DCF Handbook (10-24)

Sec. 7-35(j), Ordinance: Exits from a child care facility shall open onto safe, protected, fenced areas, or shall have active audible alert mechanisms to alert child care staff when such doors are opened. A limited exception for the front door will apply provided a child care staff member is present in the immediate vicinity of the front door at all times when the child care facility is open and caring for one or more children. An exit did not meet the requirements in that [See comments]. Comments: At the time of the inspection, the door located in the VPK classroom (room across the office) that leads to the parking lot, did not have an active audible alert mechanism.
