Sec. 402.305(4)(a), Florida Statutes: Minimum standards for the care of children in a licensed child care facility as established by rule of the department must include: 1. For children from birth through 1 year of age, there must be one child care personnel for every four children. 2. For children 1 year of age or older, but under 2 years of age, there must be one child care personnel for every six children. 3. For children 2 years of age or older, but under 3 years of age, there must be one child care personnel for every 11 children. 4. For children 3 years of age or older, but under 4 years of age, there must be one child care personnel for every 15 children. 5. For children 4 years of age or older, but under 5 years of age, there must be one child care personnel for every 20 children. 6. For children 5 years of age or older, there must be one child care personnel for every 25 children. A ratio of [see comments] child care personnel for [see comments] children is required. Ratios/Discipline [SR] Comments: At the time of arrival, two classrooms were observed out of ratio: A staff-to-child ratio of [ 1] staff for [ 6] infants/toddler & a staff-to-child ratio of [ 1] staff for [ 12] two years old. The infant/toddler and two years old classroom ratios were corrected immediately when two staff members arrived to the facility. 2.5.2(A)(1-5), Handbook: A log must be maintained for all children being transported in a vehicle or on foot away from the premises of the child care facility.The log must include each childs name; the date and time of departure; time of arrival at the destination; the signature of the driver (or in the case of travelling on foot, the signature of the child care personnel); and the signature of a second staff member or person(s) authorized by the provider to verify the transportation log and that all children have arrived safely and left the vehicle (if applicable).The facilitys log did not include all required information.
