Indoor/Outdoor Space [SR] Comments: The following repairs are needed: The caterpillar play structure was observed with a cracked siding that was on the ground and detached at entrance. The play structure (red and beige) was observed to be missing siding piece that holds up structure. Mulch needs to be added/ and or redistributed under fall zone. Compliance Comments Per director statement the swimming pool is not it use and is in need of repair. The swimming pool is covered and gated in a locked area inaccessible to the children. (Section 2.1(A) Health and Safety, Page 3) Indoor/Outdoor Space [SR] Comments: At the time of this inspection it was noted that the bottom of the second step from the ground on the climbing structure has a hole in it with a jagged edge and needs to be repaired. Sec. 7-11.10(c), Ordinance: A completed application for renewal of a license must be submitted to the local licensing agency at least 45 days prior to the expiration date of the current license to ensure that a lapse in licensure does not occur. The facility failed to submit a complete renewal application 45 days prior to the license expiration. Comments: The providers license expires on March 31/2205 and as of the time of this inspection a relicensing application has not been received at the Child Care Licensing office. Facility is reminded that the license was due in our office by February 14, 2025 and is now late and a late fee of $75.00 will now have to be paid along with the licensing fee. The facility is also reminded that if an application for relicensing is not received by Child Care Licensing office by March 17,2025 the process to deny a license for this facility will be initiated. 46. A copy of the current and approved annual fire inspection report completed by a certified fire inspector must be on file with the licensing authority. The facility has not been inspected and approved by the local fire department as required.
