7.4(F), Handbook: Prior to beginning volunteering in a child care facility, a current CF-FSP 5217, Volunteer Acknowledgment, which is incorporated by reference, must be completed and on file at the child care facility for the volunteer. A complete CF-FSP Form 5217 was not on file for all volunteers. (Section 2.1(A) Health and Safety, Page 3) Staff Requirements [SR] Comments: A complete CF-FSP Form 5217 was not on file for a volunteer who was observed working in a classroom with children. Additionally, the volunteer was not alone with the children and was working alongside a background screened staff member. An area(s) of the facility was observed to not be in good repair. (Section 2.1(A) Health and Safety, Page 3) Facility/Storage [SR] Comments: The following areas of the facility was observed to not be in good repair or cleaned: - repair the water stained ceiling (bathroom) - replace the broken stackable 3-drawer organizer - repair the peeling base boards - secure the bathroom sink to the wall - clean the presence of animal droppings (window seal of bathroom) - clean the corners in the doorway throughout the facility - clean all of the classroom spaces and indoor play areas - deep clean the floor and cabinets throughout the facility - replace the broken changing table and pad (bathroom area) - organize classroom materials - declutter all accessible areas to children throughout the facility - replace the torn naptime mats - clean the bathroom tiles - repair the bathroom cabinet - clean the ceiling and vent dust throughout the facility 11. (Section 2.1(A) Health and Safety, Page 3) Indoor/Outdoor Space [SR] Comments: The following areas of the playground was found to be in need of repairs: - repair or remove awning frame -remove all broken ride on toys, rusty play items from the playground area - repair the gap in the gate - repair the missing gate entrance - clean playground toys, rack leaves and remove trash - add additional toys, balls and other age appropriate playground equipment - Add sufficient ground covering (no dirt areas should be observed) Source: FL DCF CARES inspection 2024-11-13.
