4.2.4(B), Handbook: the facility shall maintain the following number of child care personnel with the CPR certification and first aid training that are on site at the facility at all times children are in care. For child care personnel to count to meet requirement in the below chart, they shall be child care personnel assigned to provide direct supervision of children in care: 1. For 0-25 child care personnel in direct supervision of children to meet staff to child operating ratios there must be 3 child care personnel with pediatric CPR and first aid certification, 2. For 26-50 child care personnel in direct supervision of children to meet staff to child operating ratios there must be 4 child care personnel with pediatric CPR and first aid certification, 3. Sec. 7-45(b)(5), Ordinance: A child care staff member, in addition to the driver, must be present in the vehicle when transporting children under five years of age. The nondriver child care staff member shall be seated in the vehicle in the back seat or in a position that allows: a. Each child to be seen with a quick glance; b. Each child to be heard at all times; c. Each childs activities to be observed; and d. Child care staff to respond immediately should there be an emergency. This area is out of compliance in that [see below]. Comments: At the time of inspection, transportation logs show a 3yr old child being transported in a vehicle without an additional child care staff member. 6.5(C), Handbook: Prescription medication must have a label stating the name and contact information of the physician, childs name, name of the medication, and medication directions. The label on the prescription medication did not have the required information. (Section 2.1(A) Health and Safety, Page 3) Childrens Records [SR] Comments: At the time of inspection, licensing monitor observed an injectable medication that did not contain identifying information (prescriber, dosage, child's name, medication name, etc.), a single tablet named, and a bottle of children's Motrin. Sec. 7-30(b), Ordinance: All staff members of child care facilities must have a high school diploma or the equivalent. A high school diploma or equivalent was not on file for all staff members. 14. Drinking Fountains/Water Coolers Group: Sec. 7-35, Ordinance and DCF Handbook (14-02)

Sec. 7-35(c)(2), Ordinance: Any child care facility with a licensed capacity of 50 children or fewer shall install a minimum of two drinking fountains, including at least one inside the facility and at least one in any outdoor play area. Child care facilities with a licensed capacity of more than 50 children shall have one additional drinking fountain located either inside or outside the facility for each additional 50 children licensed capacity or any fraction thereof. Manual or electrically powered commercial water coolers may be used to meet the indoor drinking fountain requirements. Electrically powered commercial water coolers may be used to meet the outdoor drinking fountain requirements. Water coolers with a hot water feature are prohibited for use by children at child care facilities. The facility has an inadequate number of working indoor/outdoor drinking fountains. An additional [1] indoor/outdoor fountain(s) is/are required. Comments: At the time of inspection, the following was observed A fountain on the playground was not observed to have running water • An electrically powered water cooler was not observed to be accessible to children. 3.9.4, Handbook: For facilities that prepare food, non-disposable food equipment, tableware, and utensils utilized for food preparation must be properly cleaned by pre-rinsing or scraping, washing, rinsing, sanitizing, and air drying. If the child care facility lacks adequate dishwashing and sanitation described in this section for dishes, equipment and utensils, only disposable single-use items may be used. All single service items must be discarded after each use. Food equipment, tableware and utensils used to prepare food must be washed and sanitized on-site except when a caterer is used, and the caterer is responsible for dishwashing as evidenced by a written agreement. Non-disposable food equipment, tableware, and utensils utilized for food preparation and food consumption were not properly washed and sanitized on-site. Food Service/Nutrition [SR] Comments: At the time of inspection, licensing monitor observed [6] juice bottles in the refrigerator that were repurposed as water bottles for serving children. Per provider, bottles are washed and refilled daily. Facility does not meet minimum sink requirements for dishwashing/sanitization. Provider resolved by removing and discarding bottles. Technical assistance: the designated meal preparation area opens to a classroom on one side and a restroom on the other.
