Sec. 7-33(e), Ordinance: There shall be at least one child care staff member providing direct supervision at all times when one or more children are present. At no time shall any children be left alone for any reason without direct supervision by a child care staff member. Direct supervision means watching and directing the childrens activities within close proximity in the same room or within a designated outdoor play area, and responding to each childs needs. Child care personnel and volunteers at a facility must be assigned to provide care to a specific group of children and be present with that group at all times during the day, including during meals, napping, and transportation of children 2.4.1(B), Handbook: Children must never be left inside or outside the facility, in a vehicle, or at a field trip location by themselves. Lack of direct supervision occurred when [see below]. Ratios/Discipline [SR] Comments: Director MP self-reported the following incident and discussed with licensing monitor the following incident: On 03/26/26 around 4pm, the 3yr and 4-5yr old classes combined on the playground for a ratio of 2:15. At 4:20pm, children were brought inside to wash hands and prepare for pickups. Shortly after, parent of 3yr old child SH came for pickup and asked child care personnel DS for the child. DS stated the child was outside on the playground. Parent went out to the playground to find 3yr SH and another child, 3yr old DA, on the playground without supervision. DS was aware that 15 children were originally in the group, but believed the other children were outside with RJ. A Notice of Violation is issued. 08. 3.9.1(B), Handbook: Child care personnel, while working in the food preparation area, must wear proper head covering, such as a hair net or hat. Child care personnel working in the food preparation area did not wear proper head covering. (Section 2.1(A) Health and Safety, Page 3) Food Service/Nutrition [SR] Comments: At the time of inspection, licensing monitor observed personnel working in the food preparation area without proper head covering. Provider resolved by placing hairnet. Technical assistance: CCF Handbook, Section 3.9.3(D), "If a facility chooses to provide food to children directly or by contract with an outside source, such as a caterer, the food must be free from spoilage and handled in a sanitary manner at all times. 6.5(H), Handbook: Medication that has expired or that is no longer being dispensed must be returned to the custodial parent or legal guardian or discarded. Expired medication or medication that is no longer being dispensed was not returned to the custodial parent or legal guardian. Childrens Records [SR] Comments: At the time of inspection, licensing monitor observed [1] epi-pen to be expired. Per provider, parents have been working with physicians to determine if epi-pen is medically necessary, but a decision has not been made and the epi-pen was not returned to parents. Epi-pen expired 10/2024. Additionally, epi-pen was stored in a Ziploc bag and not in its original container/box with appropriate labeling (child's name, dosage information, physician's information, etc.) Source: FL DCF CARES inspection 2025-03-31.
