6.4(A), (B) and (D), Handbook: All accidents and incidents or unusual occurrences that are threatening to the health, safety, or welfare of a child that occur while the child is in care must be documented on the same day they occur. This documentation must be shared with the custodial parent or legal guardian on the date of occurrence. If the parent or legal guardian does not pick up the child on the date of occurrence of the accident or incident, the individual authorized to pick up the child must sign and be provided a copy of the accident/incident form. Documentation of an accident/incident was not provided on the day it occurred. Childrens Records [SR] Comments: Complaint alleges that at around 5:00 PM that afternoon, child had a red, swollen mark near his right eye and seemed a clear gel had been applied to it. The parent ad not been contacted about any incident earlier in the day, and this visible injury was alarming. Ms. C. appeared surprised when asked what happened and stated that she was unaware of the injury. This response was inconsistent with the incident report, which later noted that staff had observed some irritation near Chaises eye. Despite this, the director informed the parent that she was not aware of any incident. On Monday, June 16, 2025, an incident report was provided containing no details about how the injury occurred. FINDINGS: A complaint inspection was conducted on 7/31/2025 and a statement from the director was received. Per the director, the parent did bring the mark to her attention. She didn't know what had happened so she went to ask the teacher. The teacher told her nothing happened. The director stated that the pastor reviewed the footage of the date in question and didn't observe anything that would have caused the injury to the child's eye. Per the teacher, the only incident she remembers is when the children were playing at the table with a toy. The child hit himself with the toy on the face. She did not see any mark on him until she was changing his diaper. When she saw the red maker, she applied Vaseline on the eye. This complaint is substantiated and a non-compliance for not providing an incident report on the date of the incident was issued. 41.
