Physical Facility [SR] Comments: At the time of the inspection, patches of peeling paint were observed within child's height on the walls of one of the classrooms. Technical assistance was provided that peeling paint is a health hazard to children. The facility is placing a maintenance order to repair the wall's peeling paint. Safety [SR] Comments: At the time of the inspection, the facility did not have written records of emergency preparedness drills. Technical assistance was given to the provider, including providing the director a copy of the DOH form recording log on emergency preparedness drills to which the provider printed and saved on file.
