    An area(s) of the facility was observed to not be in good repair. CCF Handbook, Section 3.1, A Comments: During the time of inspection, Licensing Specialist observed several ceiling tiles needing replaced that had holes and cracks and water damage in all of the classrooms. The paint on the walls were peeling or chipped in all of the classrooms. The lights were flickering and need replaced in the three-year-old and VPK classrooms. There was a hole in the window seal of the first window as well as damage to the wall under the window in the three-year-old classroom. The doorknob on the door leading to the bathroom was loose in the three-year-old classroom. There was a hole in the wall under left window and rust around handwash sink in the VPK classroom (#7). All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. CCF Handbook, section 7.4 Personnel Records Records must be maintained and kept current on all child care personnel, as defined by Section 402.302(3), F.S. These records shall be on-site, available for review by the licensing authority and must include: A. A complete employment application with the required statement pursuant to Section 402.3055(1)(b), F.S. B. Documentation of position and date of employment. C. CF-FSP Form 5337, Child Abuse & Neglect Reporting Requirements, which is incorporated by reference in 65C-22.001(7)(l), F.A.C., must be signed on or before hire date and annually thereafter by all child care personnel. D. Copies of training information and credentials as applicable. 7.4.1 Background Screening Documents Background screening documentation must be maintained for all child care personnel as defined by Section 402.302(3), F.S., which includes household members if the facility is located in or adjacent to the home of the operator. Background screening documentation must be on-site and available for the licensing authority to review. A. Each personnel record must have a completed CF-FSP Form 5131, Background Screening and Personnel File Requirements, which is incorporated by reference in paragraph 65C-22.001(7)(b), F.A.C. B.
