CCF Handbook, Section 3.1, A (Section 2.1 Health and Safety, Page 3) Physical Environment [SR] Comments: During the time of inspection, Licensing Specialist observed a large chunk of the exterior wall was broken off on the corner in the outdoor picnic area. This area is cracked, peeling and breaking off. Licensing Specialist also observed the wooden floor panels that lead to the outdoor play area to be uneven and not leveled, which could cause a tripping hazard. 13. Emergency preparedness drills were not conducted when children were in care. CCF Handbook, Section 3.8.5, B (Section 2.1 - Health and Safety, Page 3) Physical Environment [SR] Comments: During the time of inspection, a fire drill was conducted in the presence of the Licensing Specialist. There were 4 adults and 26 children present. The last fire inspection was conducted on 10/27/25. Fire extinguisher certifications are valid through 09/2026. Licensing Specialist observed that the last lockdown or inclement weather drill had been conducted on 03/2023. An inclement weather drill was conducted in the presence of the Licensing Specialist. CCF Handbook, Section 3.1, A (Section 2.1 Health and Safety, Page 3) Physical Environment [SR] Comments: During the time of inspection, Licensing Specialist observed paint peeling along the wall in the 2 three-year-old classrooms. Licensing Specialist observed the molding/trim around the door in the three-year-old classroom was damaged/broken causing the exposed wood to have sharp edges. Licensing Specialist observed the wall in the bathroom that is shared between the two- and three-year-old classrooms to have water damage, which makes it difficult to properly clean and sanitize. Licensing Specialist observed the wooden picnic table on the three-year-old playground to have large holes that children could stick their fingers into. Licensing Specialist observed play equipment (red, blue, green car) on the three-year-old playground to have peeling paint and was not in good repair. The ground cover or other protective surface under the [slide/swings] was not maintained. CCF Handbook, Section 3.12, D (Section 2.1 Health and Safety, Page 3) Sanitation and Equipment [SR] Comments: During the time of inspection, Licensing Specialist observed that the black tarp used for ground covering on the two-year-old playground had holes in it which could cause a tripping hazard. Licensing Specialist observed that the ground covering did not measure 6 inches in depth around the slides and swings. Licensing Specialist observed that the wire fence/gate was broken and had sharp edges along the back of the after schoolers playground. Licensing Specialist observed that the wood panels used for flooring in the outdoor play area were not nailed safely to ground, which caused them to lift up off of the ground when stepped on. CCF Handbook 3.12 Outdoor Equipment C. All equipment, fences, and objects on the facilitys premises shall be free from sharp, broken and jagged edges, and properly placed to prevent overcrowding or safety hazards in any one area. D. Permanent or stationary playground equipment must have a fall/use zone that extends a minimum of 6 feet in all directions from the perimeter of the equipment. All types of ground cover must be maintained to provide resilience and reduce the incidence of injuries to children in the event of falls. 1. If the ground cover in place is loose ground cover (such as, but not limited to: mulch, shredded rubber chips, or sand) a minimum of 6 inches in depth is required in the use zone. Asphalt, concrete, hard packed dirt, hay, grass or leaves are unsuitable for use in the use zone area. The Florida Certificate of immunization was not acceptable in that: [ The form was not current (expired)]. CCF Handbook, Section 7.1 (Section 2.1 Health and Safety, Page 3) Record Keeping [SR] Comments: During the time of inspection, the Licensing Specialist observed two childrens files with expired immunization records. CCF Handbook, section 7.1 Immunization Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form Part A-1, B, or C, DH 680, which is incorporated by reference in 65C-22.001(7)(o), F.A.C., or the Religious Exemption from Immunization form, DH 681, which is incorporated by reference in 65C-22.001(7)(p), F.A.C., from the custodial parent or legal guardian. DH Form 680 and DH Form 681 may be obtained from the local county health department. Specific immunization requirements are included and detailed in the most current edition of the Immunization Guidelines-Florida Schools, Child Care Facilities and Family Day Care Homes as promulgated by the Florida Department of Health. B. If the custodial parents or legal guardians fail to provide the documentation required above within 30 days of enrollment, the facility shall not allow the child to remain in the program. The parent/guardian of a child who has not received the age-appropriate immunizations prior to enrollment and who does not have documented medical or religious exemptions from routine childhood immunizations must provide documentation of a scheduled appointment or arrangement to receive immunizations. Providers must include a general statement in parent handbook/policies to inform parents/guardians, at time of enrollment, that some children in care may not have current immunizations. 42. The Licensing Specialist monitored 105 of the 105 childrens records. REMINDER: Influenza brochures must be signed annually between August 1st and September 30th for each enrolled child.
