An area(s) of the facility was observed to not be in good repair. CCF Handbook, Section 3.1, A (Section 2.1 Health and Safety, Page 3) Physical Environment [SR] Comments: During the time of inspection, the Licensing Specialist observed that the white half door leading to the two- and three-year-olds classroom has paint chipping throughout the side of the door, the yellow wall in the one- and two-year-olds bathroom has paint peeling throughout, the peach wall in the VPK bathroom has several spots with paint peeling. According to the DCF handbook: All childcare facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. Any problems must be corrected before the play area is used by children. Documentation of the indoor play area inspection must be maintained for 12 months. 13. The director responsible for the daily operation of the program did not have an active Director Credential. CCF Handbook, Section 4.7 Comments: The Licensing Specialist did not visit the facility as this is a paper inspection. The Director listed at the facility has an expired Director Credential that expired on 3/5/25. The facility will be placed on a 6 month provisional license. Per the DCF Handbook Section 402.305(2)(g), F.S., requires a child care facility to have a credentialed director. Every applicant for a license to operate a child care facility or a license for a change of ownership of a child care facility must document that the facility director has an active Director Credential prior to issuance of the license. An applicant for the Director Credential must meet the requirements referenced in CFFSP Form 5290, Florida Child Care and Education Program Director Credential and Renewal Application, which is incorporated by reference in 65C-22.001(7)(k), F.A.C. CCF Handbook, Section 3.10.2, D (Section 2.1 Health and Safety, Page 3) Sanitation and Equipment [SR] Comments: During the time of the inspection the Licensing Specialist observed the diaper changing table next to the bottle prep area. This was corrected at the time of the inspection as the Director moved the diaper changing table and put all the bottle prep items in the kitchen to be prepared. The ground cover or other protective surface under the [swings and the 2 metal climbing gyms] was not maintained. CCF Handbook, Section 3.12, D (Section 2.1 Health and Safety, Page 3) Sanitation and Equipment [SR] Comments: During the time of the inspection the Licensing Specialist observed that the fall zones under the swings and 2 metal climbing gyms had less then 1 inch of mulch under all of them. Per the DCF Handbook Permanent or stationary playground equipment must have a fall/use zone that extends a minimum of 6 feet in all directions from the perimeter of the equipment.
