The facility failed to report a serious injury or death to the licensing authority. CCF Handbook, Section 6.4, E Comments: Provider was notified by parent on February 14, 2025, with discharge paper identifying a serious injury to the wrist of child. Based on the statements provided from the provider, licensing authority was not notified of the medical findings a result an incident that took place at the facility. Interview with Owner/Director [ES]: "There were no visible signs of broken skin, bleeding, or cuts, which is why only the ice pack was applied. Based on the injury I observed, I did what I believed was best to assist the child." Technical assistance was offered to the provider per CCF handbook " In the event of serious injury or death, the incident must immediately be reported to the licensing authority." 40.
