Based on the statements provided from the provider, licensing authority was not notified of the medical findings a result an incident that took place at the facility. Interview with Owner/Director [ES]: "There were no visible signs of broken skin, bleeding, or cuts, which is why only the ice pack was applied. Based on the injury I observed, I did what I believed was best to assist the child."
