   Child care personnel failed to possess a current attendance record and parent contact information during a fire drill, emergency preparedness drill or an actual emergency. CCF Handbook Section 3.8.4, C and 3.8.5, A (Section 2.1 - Health and Safety, Page 3) Physical Environment [SR] Comments: Licensing Specialist during a Fire Drill observed that Child care personnel failed to possess a current attendance record. Licensing Specialist toke the room number from the staff with the number of children participating during the fire drill and went back to each classroom to verify the attendance roster. T.A. A current attendance record and parent contact information must accompany child care personnel out of the building during a drill or actual evacuation and be used to account for all children. The operator must maintain a written record of the fire drills showing the date, number of children and child care personnel in attendance, evacuation route used, and time taken for all individuals to evacuate the premises. Each fire drill record must be maintained for a minimum of 12 months from the date of the fire drill. The fire drills conducted must include, at a minimum: 1. One fire drill using an alternate evacuation route, 2. One fire drill during napping/sleeping times, and 3. One fire drill in the presence and at the request of the licensing authority. This drill will be coordinated with the operator or designee.    The personnel/ volunteer (ten hours or more per month) record did not include a CF-FSP 5337 Child Abuse and Neglect Reporting Requirements form signed annually. CCF Handbook, Section 7.4, C (Section 2.1 Health and Safety, Page 3) Record Keeping [SR] Comments: Licensing Specialist reviewing child care personnel found that not all child care personnel have on file a CF-FSP 5337 Child Abuse and Neglect Reporting Requirements form signed annually. Per Provider the Child Abuse and Neglect Reporting Requirements was update at the time of the inspection for all child care personnel. T.A. CF-FSP Form 5337, Child Abuse & Neglect Reporting Requirements, which is incorporated by reference in 65C-22.001(7)(l), F.A.C., must be signed on or before hire date and annually thereafter by all child care personnel. All (34) child care personnel records in the system were reviewed of which (18) were updated for compliance. 1 new personnel were hired since the last inspection as per the director, and 2 new personnel added since the last inspection. 45.
