   A sleep sack used did not fit according to manufacturers recommendations, restricted the infants arms, or slid up around the infants face. CCF Handbook, Section 3.6.4, B Physical Environment [SR] Comments: At time of inspection, the counselor observed an infant swaddled in a sleep sack that restricted the infant's arms. The infant was placed in the bouncer swaddled. The counselor advised the provider that the sleep sack was inappropriate because it restricted the infant's arms. The counselor advised the provider of the appropriate sleep sack, that can be used for children in care. The staff immediately removed the child at the time of inspection.
