Documentation of pediatric cardiopulmonary resuscitation certification was not on file for child care personnel. SACCF Handbook Section 4.2.2, A (Section 2.1 Health and Safety, Page 3) Health Requirements [SR] Comments: There are currently 5 employees. All 5 employee records were reviewed. The Director, ACs CPR/First Aid certification has expired. At this time, the requirements for this standard are not meeting met. Upon arrival there were only 2 employees present and each person was in charge of a different classroom. A third staff person did arrive later in the inspection. The School Age Child Care Handbook states on pages 36-37 that: 4.2.2 First Aid and Cardiopulmonary Resuscitation (CPR) A. Each school-age child care program must have at least one child care personnel with a current and valid certificate(s) of course completion for first aid training and pediatric cardiopulmonary resuscitation (CPR) procedures. By June 12, 2021, at all times in each classroom/room/designated space, or outdoor area occupied by children, at least one of the child care personnel School-Age Child Care Facility Handbook | P a g e 36 assigned to provide direct supervision to that specific group must have current first aid training and pediatric cardiopulmonary resuscitation (CPR) certification. Documentation of first aid training and pediatric cardiopulmonary resuscitation (CPR) certification must be kept on file and available for licensing to review. B. One child care personnel satisfying these training requirements must be present at all times that children are in care at the facility, on field trips, which includes all activities away from the program, and during all transportation activities. In the event a group of children are away from the program on a field trip and other children remain at the facility, there must be one child care personnel satisfying these training requirements present at each location at all times, both on-site and on the field trip. C. Certificates of course completion are valid based on the time frames established by each first aid and CPR training program, not to exceed three years. D. CPR courses must include on-site, instructor-based skill assessments by a certified CPR instructor. Documentation of completion of the online course and on-site assessment must be maintained at the facility and available for review by the licensing authority. 04. Supervision SACCF Handbook, Section 2.4 (04-08)

In addition to the child care personnel required to meet staff-to-child ratios, one additional adult was not present during a field trip. SACCF Handbook, Section 2.4, H General Requirements [SR] Comments: The Licensing Specialist observed two vans that are used by the facility to transport, returning from a field trip. In each van was a staff member and 14 children. There was not an additional adult attending the field trip. The Licensing Inspector discussed this standard with the Director. The Director made arrangements with their other facility to have an additional staff person attend each field trip moving forward. The School Aged Child Care Facility Handbook states on page 12 that: H. In addition to the number of child care personnel required to meet the staff-to-child ratio, for the purpose of safety, one additional adult must be present on all field trips away from the school-age child care program to assist in providing direct supervision. The individual could be a parent volunteer as long as that person is under direct and constant supervision of a screened and trained child care personnel of the school-age child care program. Compliance Comments The facility does provide transportation. There are two vans that are used to transport. The Licensing Specialist reviewed the transportation logs for the program. 23. Food Safety SACCF Handbook, Section 3.9.3 (23-21)

The facility's menu was not [ was not posted at the beginning of each week in an easily seen place]. SACCF Handbook, Section 3.9.3, G Food and Nutrition [SR] Comments: During the inspection, the menu posted at the facility was dated September 2024. The School-Age Child Care Facility Handbook states on page 31 that: G. Meal and snack menus must be planned, written, dated, and posted at the beginning of each week in an easily seen place accessible to parents. Any menu substitution must be noted on the menu. A generalized menu of possible snack choices for programs that receive food donations is acceptable. All meals and snacks prepared outside of the facilitys kitchen or designated food preparation area, such as catered food, must be listed along with the source. Daily meal and snack menus must be maintained for a minimum of 12 months for licensing purposes. Operators who participate in the USDA Food Program must keep menus in accordance with the Department of Health and USDA requirements. 24.
