CCF Handbook, Section 3.12, D (Section 2.1 Health and Safety, Page 3) Sanitation and Equipment [SR] Comments: The facilitys playground has a swing set and several other playground pieces that are anchored in and need to meet the fall zone requirements, The Director reported that mulch will be ordered. The Child Care Facility Handbook states on pages 38-39 that: D. Permanent or stationary playground equipment must have a fall/use zone that extends a minimum of 6 feet in all directions from the perimeter of the equipment. All types of ground cover must be maintained to provide resilience and reduce the incidence of injuries to children in the event of falls. 1. If the ground cover in place is loose ground cover (such as, but not limited to: mulch, shredded rubber chips, or sand) a minimum of 6 inches in depth is required in the use zone. Child care personnel did not complete preservice training requirements prior to unsupervised contact with or care of children or, if supervised, within 90 days of beginning employment or service with the provider. (Section 3.1 and 3.2 Training Requirements, Page 7) [SR] Comments: There are currently 12 employees. All 12 employee records were reviewed. One employee, KP, has been at the facility for over 90 days and has not completed the School Readiness pre service training requirements. The School Readiness Program Health and Safety Standards Handbook states on page 7 that: 3 Training Requirements Training requirements of this section do not apply to child care personnel who do not work directly with children, such as cooks, bookkeepers, and janitors who are considered other personnel. Training is required for other personnel if they at any time fill in for classroom child care personnel and are in direct care of children. 3.1 Preservice Child care personnel must successfully complete the preservice training coursework described in this section, as applicable. All child care personnel must complete these preservice training requirements within 90 days of initial employment with any School Readiness provider. This timeframe does not start over if personnel change employment to another School Readiness provider within this initial 90 days. The facility did not have a fully-completed enrollment form (CF-FSP Form 5219) or equivalent form for the child(ren) in care. CCF Handbook, Section 7.3 (Section 2.1 Health and Safety, Page 3) Record Keeping [SR] Comments: Twenty newly enrolled children's records were reviewed since the last inspection on 6/17/2025. One child, GH, did not have a parent signature or date on the completed child care application on file. The Child Care Facility Handbook states on page 61 that: 7.3 Enrollment Information The facility operator shall obtain enrollment information from the childs custodial parent or legal guardian prior to accepting a child in care. This information shall be documented on CF-FSP Form 5219, Child Care Application for Enrollment, which is incorporated by reference in 65C-22.001(7)(f), F.A.C., or an equivalent form that contains all the information required by the Department on CF-FSP Form 5219. CF- FSP Form 5219 may be obtained from the licensing authority or on the Departments website at www.myflfamilies.com/childcare. A. Enrollment information shall be kept on file, current and available for review by the licensing authority. B. The enrollment information shall include, in writing, permission for the facility to release the child to any person(s) authorized or in the manner authorized by the custodial parent or legal guardians. The name, address, and phone number of authorized persons must be in the enrollment information. C. There shall be signed statements from the custodial parents or legal guardian that the child care facility has provided them with the following information: 1.
