Child Care personnel, who were not in compliance with training requirements when they left the industry, did not complete required training and any new mandated training before returning to the industry. CCF Handbook, Section 3.9.2, E (Section 2.1 Health and Safety, Page 3) Food and Nutrition [SR] Comments: Specialist observed there were open boxes and no dates when they were open for the children. Specialist had the director put dates on all the boxes that were opened. It was completed at inspection. Per childcare facility handbook on page 30. The base of the fence in the outdoor play area was not at ground level and could allow inside or outside access by children or animals. CCF Handbook, Section 3.5, F (Section 2.1 Health and Safety, Page 3) Physical Environment [SR] Comments: Specialist observed that the fence does not go to the ground and wood piece in front of fence needs to be fixed or replaced, for safety of the children and so animals cannot get on to playground. Per the childcare facility handbook on page 24. The outdoor play area must have adequate fencing or walls a minimum of 4 feet in height. Fencing, including gates, must be continuous and must not have gaps or opening larger than 3 inches that would allow children to exit the outdoor play area. The base of the fence must remain at ground level and be free from erosion or build up to prevent inside and outside access by children or animals. The personnel/ volunteer (ten hours or more per month) record did not include a CF-FSP 5337 Child Abuse and Neglect Reporting Requirements form signed annually. CCF Handbook, Section 7.4, C Record Keeping [SR] Comments: Specialist observed that the staff's Child Abuse and Neglect Reporting Forms were signed more than a year ago. Updated forms are needed. Director had all staff sign a new form during this inspection. The Child Care Facility Handbook states on page 61: C. Supervision of children in the [2-4] group was inadequate in that [The supervision in the two- four year old classroom was inadequate the children were leaving the playground to go to the restroom without a staff member present. ]. CCF Handbook, Section 2.4 General Requirements [SR] Comments: Specialist observed children coming in from playground to use the rest room with no supervision in the rest room. Director stated she supervises the children when using the rest room. Director took the children back to the playground, when children need to use the rest room, they will come in as a class to use rest room and have the supervision needed.
