There was evidence that smoking occurred in the childrenscenter, outdoor area, during a field trip or in a vehicle used to transport children while children were in care. Indoor and outdoor play areas were not inspected daily for basic health and safety and/or any problems were not corrected before the play area is used by children and/or daily inspection logs were not maintained for 12 months.
