An area(s) of the facility was observed to not be in good repair. CCF Handbook, Section 3.1, A (Section 2.1 Health and Safety, Page 3) Physical Environment [SR] Comments: During the inspection, Licensing Specialist observed on the vehicle the seat belts to the booster seats the black buckle that goes across the chest does not stay snapped. The trim is broken and has sharp pieces along the back door in the following classrooms: older 1 year- olds, 2 year-olds, and P2. The flooring in the bathrooms is peeling up in the following classrooms: VPK and the 3/4 year-olds. In the VPK classroom in the very back the base board is missing in places. The director was given until March 20, 2026, to come into compliance with this standard. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook Section 3.1 A.- General Health and Safety Requirements. A. All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. Any problems must be corrected before the play area is used by children. Documentation of the indoor play area inspection must be maintained for 12 months. 13. The facilitys outdoor play area contained litter, nails, glass or other hazards that posed a low potential for harm to children. CCF Handbook, Section 3.5, A (Section 2.1 Health and Safety, Page 3) Physical Environment [SR] Comments: During the inspection, Licensing Specialist observed on all playgrounds the jungle gyms had a black substance and white and green fuzzy substances on them. On the school age playground the pillar that is connected to the building the bottom of the pillar the stucco is broken off. The base of the fence in the outdoor play area was not at ground level and could allow inside or outside access by children or animals. CCF Handbook, Section 3.5, F (Section 2.1 Health and Safety, Page 3) Physical Environment [SR] Comments: During the inspection, Licensing Specialist observed on the school age playground the fence that is behind the basketball court has a gap bigger than 3 1/2 inches. The gap measured at 4 1/2 inches in length and 7 inches in width. The director was given until March 20, 2026, to come into compliance with this standard. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook Section 3.5 F- Outdoor Play Area. F. The outdoor play area must have adequate fencing or walls a minimum of 4 feet in height. Fencing, including gates, must be continuous and must not have gaps or opening larger than 3 inches that would allow children to exit the outdoor play area. The base of the fence must remain at ground level and be free from erosion or buildup to prevent inside and outside access by children or animals. These areas must have at least two exits, with at least one being remote from the buildings. If an outdoor play area was approved for usage by the Department prior to October 25, 2017, no new exits are required to be added to meet this standard. A prescription or non-prescription medication, specifically [Boudreau Butt Paste, Diaper Butter Paste, Aquafer, Babo Botanicals,], was dispensed without written authorization from the custodial parent or legal guardian. CCF Handbook, Section 6.5, A and K (Section 2.1 Health and Safety, Page 3) Health Requirements [SR] Comments: During the inspection, Licensing Specialist observed the medication forms were incomplete in that they were missing the following components: time and amount of the correct dosage to be given. The director was given until March 2, 2026, to come into compliance with this standard. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook Section 6.5 Medication A-E : Child care programs are not required to give medication; however, if a program chooses to do so, it must comply with the following requirements: A. The child care program must have written authorization from the custodial parent or legal guardian to give prescription and non- prescription medications. This authorization must be dated and signed by the custodial parent or legal guardian and contain the childs name; the name of the medication to be given; and date, time and amount of the correct dosage to be given. Prescription and non- prescription medications that are used on an as needed basis require the parent/ legal guardian to provide additional documentation on the authorization form to describe symptoms that would require the medication to be given. A fire hazard, [dust build up in the vents], was observed in the facility. CCF Handbook, Section 3.1, B (Section 2.1 Health and Safety, Page 3) Physical Environment [SR] Comments: During the inspection, Licensing Specialist observed a buildup of dust on the vents in the P3/P2 bathroom. The director brought this standard back in compliance at the time of inspection by cleaning the vents. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook Section 3.1 B.- General Health and Safety Requirements. B. It is the responsibility of the director/operator to ensure all areas and equipment of the facility are free from fire hazards, such as lint build up in heating and air vents, filters, exhaust fans, ceiling fans, and dryer vents. The facility did not have electrical outlet covers or the outlet covers were not in place. CCF Handbook, Section 3.1, I (Section 2.1 Health and Safety, Page 3) Physical Environment [SR] Comments: During the inspection, Licensing Specialist observed a power strip that was on the right side of the computer 3 outlets did not have outlet covers. The director brought this standard back in compliance at the time of inspection by adding outlet covers to the power strip. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook Section 3.1, I.- General Health and Safety Requirements. - I. All accessible electrical outlets must be tamper-resistant electrical outlets that contain internal shutter mechanisms to prevent children from sticking objects into receptacles. The floor mats available for children in care were not covered with an impermeable surface. CCF Handbook, Section 3.6.1, D (Section 2.1 Health and Safety, Page 3) Physical Environment [SR] Comments: During the inspection, Licensing Specialist observed in the VPK, P3 and Preschool 3 classrooms the mats were peeling. The owner stated she has cots ordered but the cots are on back order. Since the order is on back order the specialist is giving them extra time to bring this into compliance. The director was given until May 14, 2025, to come into compliance with this standard. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook Section 3.6.1,D- Bedding and Linens. Each child in care must be provided safe and sanitary bedding to be used when napping or sleeping. Nap bedding is not required for school-age children; however, the program must provide an area for children that choose to rest. D.
