32. Outdoor Equipment CCF Handbook, Section 3.12 (32-10)

A wading pool, or inflatable water slide with a landing area where water collects, was used by children in care. CCF Handbook, Section 3.12, H Comments: 32-10: During the inspection, Licensing Specialist observed that there was an inflatable pool water side with kiddie pools being stored on the playground. Staff advised that they used the pools and water slide for water day on Fridays (weather permitting). During the inspection, the inflatable pool was removed. This is being marked corrected at the time of the inspection. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 3.12, H Wading pools, including inflatable water slides with a landing area where water collects, are prohibited. TRAINING Compliance Comments During the time of inspection, the Owner/Operator/Director advised the Licensing Specialist that there 20 employees. During the time of the inspection, the Licensing Specialist monitored 20 of the 20 employee records. REMINDER: In-service training for 2025/2026 fiscal year must be completed before July 1, 2026. CCF Handbook, Section 6.4, C Comments: 39-5: During the inspection, Licensing Specialist observed that some of the accident and incident reports were missing information, such as, the time of the incident/accident. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 6.4, C Documentation must include the name of the affected party, date and time of the occurrence, description of the occurrence, actions taken and by whom, and appropriate signatures of program child care personnel and the custodial parent or legal guardian. The label on the prescription medication did not have the required information in that it did not include [ name and contact information of the physician and/or pharmacy, childs name, name of the medication, and medication directions. ]. CCF Handbook, Section 6.5, C Comments: 40-5: During the inspection, Licensing Specialist observed 1 medication (Mupirocin) that did not have the doctors script that included name and contact information of the physician and/or pharmacy, childs name, name of the medication, and medication directions. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 6.5, C Prescription and non- prescription medication brought to the child care facility by the custodial parent or legal guardian must be in the original container. Prescription medication must have a label stating the name and contact information of the physician and/or pharmacy, childs name, name of the medication, and medication directions. All prescription and non-prescription medication must be dispensed according to written directions on the prescription label or printed manufacturers label and maintained at the appropriate temperature. An area(s) of the facility was observed to not be in good repair. CCF Handbook, Section 3.1, A Comments: During the inspection, Licensing Specialist observed in YCP1 classroom there is 2 holes in the wall under the window in the library center. The director was given until April 9, 2026, to come into compliance with this standard. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook Section 3.1 A.- General Health and Safety Requirements. A. All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. Any problems must be corrected before the play area is used by children. Documentation of the indoor play area inspection must be maintained for 12 months. A fire hazard, [dust build up on the vents], was observed in the facility. CCF Handbook, Section 3.1, B Comments: During the inspection, Licensing Specialist observed a buildup of dust on the vents in the bathrooms in the following classrooms: PP1 and PP2. The director brought this standard back in compliance at the time of inspection by cleaning the vents. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook Section 3.1 B.- General Health and Safety Requirements. B. It is the responsibility of the director/operator to ensure all areas and equipment of the facility are free from fire hazards, such as lint build up in heating and air vents, filters, exhaust fans, ceiling fans, and dryer vents. The facilitys outdoor play area contained litter, nails, glass or other hazards that posed a low potential for harm to children. CCF Handbook, Section 3.5, A Comments: During the inspection, Licensing Specialist observed on PP1 and PP2 on the jungle gyms there is a green fuzzy substance, there is rust on the metal poles with yellow tape wrapped around the poles and the wooden post that goes around the jungle gym at the bottom it is starting to deteriorate. On the PP2 playground the bottom of the window/mirror on the jungle gym is falling down and exposing a screw. The director was given until April 9, 2026, to come into compliance with this standard. The ground cover or other protective surface under the [jungle gyms] was not maintained. CCF Handbook, Section 3.12, D Comments: During the inspection, Licensing Specialist observed on the playground the mulch under the fall zones were not 6 inches in depth. On PP1 playground you were able to see the ground and on the PP2 playground the mulch is measuring at 2 inches. The director was given until April 9, 2026, to come into compliance with this standard. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook Section 3.12.D.1- Outdoor Equipment. D. Permanent or stationary playground equipment must have a fall/use zone that extends a minimum of 6 feet in all directions from the perimeter of the equipment. All types of ground cover must be maintained to provide resilience and reduce the incidence of injuries to children in the event of falls. Child Care Facility Handbook Page | 39 1. If the ground cover in place is loose ground cover (such as, but not limited to: mulch, shredded rubber chips, or sand) a minimum of 6 inches in depth is required in the use zone. 40. Medication CCF Handbook, Section 6.5 (40-22)

Additional documentation describing the symptoms that would require the prescription and/or non-prescription medication used on an as needed basis to be administered was not on file. CCF Handbook, Section 6.5, A and 2.5.3, C & D Comments: During the inspection, Licensing Specialist observed that the medication forms for diaper ointment cream did not state how much to apply on the child and when to apply. It did not state what to look for. The director brought this standard back in compliance at the time of inspection by having the parents complete the form when the parent dropped off their child. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 6.5 A. Medication. Child care programs are not required to give medication; however, if a program chooses to do so, it must comply with the following requirements: A. The child care program must have written authorization from the custodial parent or legal guardian to give prescription and non- prescription medications. This authorization must be dated and signed by the custodial parent or legal guardian and contain the childs name; the name of the medication to be given; and date, time and amount of the correct dosage to be given. Prescription and non- prescription medications that are used on an as needed basis require the parent/ legal guardian to provide additional documentation on the authorization form to describe symptoms that would require the medication to be given. The facility's outdoor play area contained litter, nails, glass or other hazards that posed a threat to the health, safety or well-being of the children. CCF Handbook, Section 3.5, A Comments: During the inspection, the Licensing Specialist observed that the fence in between the Pre-Primary 1 and 2 classrooms had sharp and rusted areas along the fence. The fence closest to the entrance near the street had 3 screw holders (two on the entrance gate, and one on the secondary entrance gate towards the back of the playground) that were rusted and had pieces coming from them. During the inspection, there were pictures taken. This standard was brought back into compliance at the time of the inspection in that two of the screw holders were repaired, and one was replaced. The fence where there were sharp and rusted areas was repaired before the second day of the inspection. During the inspection, the Licensing Specialist provided technical assistance. CCF Handbook, Section 3.12.C: Outdoor Equipment C: All equipment, fences, and objects on the facilitys premises shall be free from sharp, broken and jagged edges, and properly placed to prevent overcrowding or safety hazards in any one area. The floor mats available for children in care were not covered with an impermeable surface. CCF Handbook, Section 3.6.1, D Comments: During the inspection, the Licensing Specialist observed that in the Young Children's 1, Young Children's 2, Pre-Primary 2, and Nido classrooms there were four floor mats that had permeable surfaces that exposed the foam. During the inspection, there were pictures taken. This standard was brought back into compliance at the time of the inspection in that the staff replaced the floor mats that exposed the foam. During the inspection, the Licensing Specialist provided technical assistance. CCF Handbook, Section 3.6.1: Bedding and Linens: Each child in care must be provided safe and sanitary bedding to be used when napping or sleeping. Nap bedding is not required for school-age children; however, the program must provide an area for children that choose to rest. Comments: Ratio: Nap Time Ratio 2:11 2 Infants and 9(1 year old) 2:16 2/3-year olds 3:16 2/3-year olds 2:20- 3-6 year olds 2:19- 3-6 year olds 4 other personnel During the inspection, Licensing Specialist observed 2 childcare personnel with 2 infants and 9(1-year-olds). There was one childcare personnel using the restroom, and one was in the kitchen. The director brought this standard back in compliance by having the childcare personnel that was using the restroom and, in the kitchen, come back into the room at the time of inspection.
