Child care personnel did not complete preservice training requirements prior to unsupervised contact with or care of children or, if supervised, within 90 days of beginning employment or service with the provider. (Section 3.1 and 3.2 Training Requirements, Page 7) [SR] Comments: During the time of inspection, the Operator advised the Licensing Specialist that there 19 employees. During the time of the inspection, the Licensing Specialist monitored 19 of the 19 employee records. When the Licensing Specialist was reviewing the files the licensing specialist observed two personnel did not their School Readiness training within the 90 days. The director was given until October 4, 2025, to come into compliance with this standard. During the inspection, Licensing Specialist provided technical assistance, School Readiness Program Health and Safety Standards Handbook Section 3.1 Preservice. Child care personnel must successfully complete the preservice training coursework described in this section, as applicable. All child care personnel must complete these preservice training requirements within 90 days of initial employment with any School Readiness provider. This timeframe does not start over if personnel change employment to another School Readiness provider within this initial 90 days. Personnel who have not completed all preservice training requirements are not allowed any unsupervised contact with or care of children in a School Readiness program. CCF Handbook, Section 2.5.1 General Requirements [SR] Comments: The director advised that there are 4 drivers. Driver 1s license will expire on 6/1/29, the physical was conducted on 7/16/24, the CPR and First Aid certificate is valid until 2/28/27 and the driver has completed the transportation training. Driver 2s license will expire on 10/15/26, physical was conducted on 8/6/24, the CPR and First Aid certificate is valid until 11/30/25 and the driver as completed transportation safety training. Driver 3s license will expire on 12/23/29, physical was conducted on 3/19/2024, the CPR and First Aid certificate is valid until 11/30/25 and the driver as completed transportation safety training. Driver 4s license will expire on 9/16/26, physical was conducted on 1/31/24, the CPR and First Aid certificate is valid until 11/30/25 and the driver as completed transportation safety training. During the inspection, Licensing Specialist observed that 2 of the 4 drivers had an expired physical. The director was given until May 31, 2025, to come into compliance with this standard. The vehicle alarm installed was not in working condition. CCF Handbook, Section 2.5.4 General Requirements [SR] Comments: During the inspection, the director advised that currently there are two transportation vehicles. First vehicle is a 2007 Chevy Midwest (133372) that was last inspected on 12/26/24 and is insured through Ace American Insurance Company with an effective date of 4/1/25. The second vehicle is a 2024 Chevy Bus (186782) that was last inspected on 4/15/25 and is insured through Ace American Insurance Company with an effective date of 4/1/25. 7-06 During the inspection, Licensing Specialist observed the 2007 Chevy Midwest (133372) that the installed vehicle alarm was not in working condition in that it could not be heard from the distance of 500 feet from the vehicle. The director was given until June 22, 2025, to come into compliance with this standard. During the inspection, Licensing Specialist provided technical assistance, regarding the maintaining of the vehicle alarm. CCF Handbook, Section 2.5.4, 1,D By January 1, 2022 all vehicles used by child care facilities to transport children must be equipped with a reliable alarm system approved by the Department which prompts the driver to inspect the vehicle for children before exiting the vehicle. 1. Documentation of Level 2 Clearinghouse screening clearance was missing for child care personnel. CCF Handbook, Section 7.4.1,C Record Keeping [SR] Comments: Care Provider Background Screening Clearinghouse: The Background Screening Clearinghouse will generate e-mails and send you, the provider, a reminder at 60, 30, and 21 days prior to the retention date expiration. Please note, you will only receive that notification for the staff that are on your facility/home roster. Please keep your roster current in the Clearinghouse to avoid confusion on expiration dates, or unwanted notifications. During the inspection, the Licensing Specialist reviewed the Background Screening Clearinghouse Rooster. During the inspection, Licensing Specialist observed one personnel did not have a cleared background screening that was working. The director put the personnel on leave until she had a cleared background screening which at the time of the inspection the director sent the personnel to have her fingerprints completed. The back ground screening is stating A new screening is Required. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 7.4.1, C Background Screening Documents Background screening documentation must be maintained for all child care personnel as defined by Section 402.302(3), F.S., which includes household members if the facility is located in or adjacent to the home of the operator. Background screening documentation must be on-site and available for the licensing authority to review. C. A copy of the eligible results, for the Level 2 screening, generated from the Clearinghouse must be on record for each personnel. If awaiting Out-of-State criminal history results, a copy of the DCF email informing of the individuals eligibility for a provisional hire status must be in the personnel file.
