CCF Handbook, Section 2.4 Comments: 4-01: On 4/15/2026, Licensing Specialist observed the recording of the Manatee classroom from 4/13/2026. Licensing Specialist observed the classrooms back door being held open by teacher 1 and that there are 3 children sitting in front of the door waiting to go outside. Two of the children go outside and the child is still sitting on the floor in the classroom. At the 16 second mark, teacher 1 walks away from the door and the door closes. At the 58 second mark, the child stands up and is looking at the window and then looks around. At the 1 minute 52 second mark, the child walks over and stands in front of the window. The window is tinted so that you cannot see into the classroom from outside. At the 2 minute 37 second mark, the child walks away from the window and stands by a toy shelf. At the 3 minute 11 second mark, the child walks over to another window in the classroom. At the 3-minute 25-minute mark, the child starts playing with a toy. At the 3 minute 35 second mark, the seals teacher is seen looking through the window from the Seals classroom. At the 3 minute 51 second mark, the Manatee outside door opens and teacher 1 and teacher 2 and the child goes outside with the class. This is being marked corrected at the time of the inspection. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook Section 2.4 A. Child care personnel must position themselves in the outdoor play area so that all children can be observed and directly supervised. B. CCF Handbook, Section 2.5.2 Comments: During the inspection, the director advised that currently there are two transportation vehicles. First vehicle was not on site to be monitored. The second vehicle is a 2011 Chevy Express (135829) that was last inspected on 7/27/2024 and is insured through Progressive Express Insurance Company with an effective date of 6/1/2025. During the inspection, Licensing Specialist observed that the transportation log was missing the second childcare personnel signatures on 6 entries dated between 6/5/2025 through 7/9/25. The director was given until August 10, 2025, to come into compliance with this standard. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook section 2.5.2 Transportation Log -A. A log must be maintained for all children being transported in a vehicle or on foot away from and/or to the premises of the childcare facility. The log must be retained on file at the facility for a minimum of 12 months and available for review by the licensing authority. The log must include: 1. Each childs name, 2. The date and time of departure, 3. Time of arrival at the destination, 4. The signature of the driver (or in the case of travelling on foot, the signature of the child care personnel), and 5. The signature of a second child care personnel or person (s) authorized by the provider to verify the transportation log and that all children have arrived safely and left the vehicle (if applicable). Compliance Comments The director advised that there are 2 drivers. Driver 1s license will expire on 12/20/2031, the physical was conducted on 10/15/2024, the CPR and First Aid certificate is valid until 6/30/2027 and the driver has completed the transportation training. Driver 2s license will expire on 5/19/2032, physical was conducted on 1/9/2025, the CPR and First Aid certificate is valid until10/31/26 and the driver as completed transportation safety training. 06. 17. Outdoor Play Area/Fencing CCF Handbook, Section 3.5 (17-07)

The base of the fence in the outdoor play area was not at ground level and could allow inside or outside access by children or animals. CCF Handbook, Section 3.5, F Comments: During the inspection, the Licensing Specialist observed on the playground fence on the young playground (the playground to the far right when facing the playground) had a hole at the base of the fence larger than 3 1/2 inches deep. The whole was on the right side of the playground (when facing the playground), on the left side of the sprinkler system. The sprinkler was in the whole. The whole was 9 inches deep from the bottom of the fence to the bottom of the whole, and 10 inches wide coming away from the fence. On the older children's playground (the playground to the far left when facing the playground), there is a whole on the left fence on the left side of the playground at the bottom of the fence towards the back of the playground. The whole was 10 inches deep, and 10 inches wide when measuring along the fence. The director was given until April 10, 2025, to come into compliance with this standard. During the inspection, the Licensing Specialist provided technical assistance, CCF Handbook Section 3.5 F- Outdoor Play Area. F. The outdoor play area must have adequate fencing or walls a minimum of 4 feet in height. Fencing, including gates, must be continuous and must not have gaps or opening larger than 3 inches that would allow children to exit the outdoor play area. The base of the fence must remain at ground level and be free from erosion or build- up to prevent inside and outside access by children or animals. These areas must have at least two exits, with at least one being remote from the buildings. If an outdoor play area was approved for usage by the Department prior to October 25, 2017, no new exits are required to be added to meet this standard. The facility's transportation log did not include [drivers signature, the secondary signature, the time on the vehicle and the time off the vehicle, some had children check marked on the vehicle, but not when they got off the vehicle, and some had children checked off the vehicle but not checked on the vehicle.]. CCF Handbook, Section 2.5.2 Comments: 5-4: During the inspection, Licensing Specialist observed some transportation log entries were missing the drivers signature, the secondary signature, the time on the vehicle and the time off the vehicle, some had children check marked on the vehicle, but not when they got off the vehicle, and some had children checked off the vehicle but not checked on the vehicle. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 2.5.2 A. A log must be maintained for all children being transported in a vehicle or on foot away from and/or to the premises of the child care facility. The log must be retained on file at the facility for a minimum of 12 months and available for review by the licensing authority. The log must include: 1. Each childs name, 2. The date and time of departure, 3. Time of arrival at the destination, 4. The signature of the driver (or in the case of travelling on foot, the signature of the child care personnel), and 5. The signature of a second child care personnel or person(s) authorized by the provider to verify the transportation log and that all children have arrived safely and left the vehicle (if applicable).
